Accuracy of FibroScan in analysis of liver fibrosis in patients with concomitant chronic Hepatitis B and nonalcoholic fatty liver disease

Abstract: Liver steatosis could affect the accuracy of FibroScan in patients with chronic hepatitis B (CHB) and nonalcoholic fatty liver disease (NAFLD). This study aimed to assess the accuracy and cut-off values of FibroScan for diagnosing liver fibrosis and cirrhosis in patients with concomitant CHB and NAFLD. A total of 116 patients with concomitant CHB and NAFLD who underwent FibroScan test and liver biopsy were retrospectively enrolled. Liver fibrosis was staged according to the METAVIR scoring system. … Show more

“…The ratio of the interquartile range (IQR) of liver stiffness to the median (IQR/M) was calculated and we took the better quality criteria (10 valid measurements, IQR/M < 30%) of FibroScan to analysis. 26,27 FIB-4 was calculated using the following formula: (age [years] × aspartate aminotransferase [AST; U/L])/(platelet [10 9 /L] × (alanine aminotransferase [ALT; U/L]) 1/2 ). 14 The AST, ALT and bilirubin were checked from serum just before operation.…”

“…Transient elastography(TE), which is widely used, can accurately evaluate the inflammation and fibrosis of liver tissue. 8 , 10 Relevant research results suggest that Fibroscan may improve the sensitivity of the diagnosis of liver fibrosis in patients with CHB and ALT levels <2 times the upper normal limit, and that this sensitivity may increase with the progression of liver fibrosis. 17 However, the LSM is affected by inflammation, congestion, cholestasis, different probes, and other factors.…”

“…For example, the aspartate aminotransferase‐ (AST‐) to‐platelet ratio index (APRI) and fibrosis‐4 (FIB‐4) are scores showing satisfactory performance to exclude liver cirrhosis. 9 , 10 , 11 Recently, the gamma‐glutamyl transpeptidase‐ (GGT‐) to‐platelet ratio (GPR) has been shown to have the same diagnostic value as APRI and FIB‐4 in assessment of liver fibrosis with CHB, and even it is considered that GPR is more accurate than APRI and FIB‐4 in evaluating liver fibrosis in patients with CHB. 12 , 13 However, there are few data to explore the diagnostic value of GPR for liver fibrosis in patients with concomitant CHB and NAFLD.…”

“…Chronic hepatitis B is the most common chronic liver disease in China. For chronic hepatitis B patients with steatosis, the risks of cirrhosis and hepatocellular carcinoma are increased (15,16). It is important to screen for steatosis in patients with chronic hepatitis B.…”
