Acne-associated spondylarthropathy: radiographic features.

Abstract: Experience with six patients with severe acne and associated axial and peripheral arthritis is described. Four of the patients had a dermatologic triad of severe acne, hidradenitis suppurativa, and dissecting cellulitis of the scalp, the so-called follicular occlusion triad. All were black men with episodic peripheral arthropathy and low back pain. One had inflammatory bowel disease. Rheumatoid factor and HLA-B27 were absent in five patients who had these determinations. An erosive and proliferative arthritis … Show more

“…In contrast to other types of seronegative forms of SpA, an association with the histocompatibility antigen HLA-B27 in HS patients does not seem to exist [22,27,32] . Importantly, we did not observe any association between body height, body weight, BMI, smoking habits and the appearance of back pain or SpA, suggesting no major influence of lifestyle on the occurrence of back pain and SpA.…”

“…This initial description from 1982 was followed by a number of case reports and few small case series [21,[23][24][25][26] . The observed radiographic alterations in HS patients were described as indistinguishable from those of other seronegative forms of SpA [27] . Our precise MRI analyses [28,29] disclosed a surprisingly high number of typical changes suggesting SpA-related sacroiliitis in 56.5% of the patients.…”

“…The history, laboratory data and radiographs were reviewed and compared with 20 other cases previously reported.2-' Four additional cases were excluded because data presented was inadequate." Three cases with acne and well defined rheumatological syndromes, such as, Reiter's, (2) Crohn's disease (5) and human immunodeficiency virus infection (5) were also excluded.…”

“…Axial skeletal involvement includes asymmetric paravertebral ossification of upper vertebrae and uni-or bilateral sacroiliac fusion. Peripherally, asymmetric erosive lesions and subluxation of any joints of the hands and feet may occur, including the distal interphalangeal joints, as well as the limbs (Ellis, 1987). In consequence, some suggest this form of arthropathy in African cases to be indistinguishable from certain cases of psoriatic arthritis or Reiter's disease.…”
