Acute kidney injury due to bilateral ureteral obstruction in children

Abstract: Bilateral ureteral obstruction in children is a rare condition arising from several medical or surgical pictures. It needs to be promptly suspected in order to attempt a quick renal function recovery. In this paper we concentrated on uncommon causes of obstruction, with the aim of giving a summary of such multiple, rare and heterogeneous conditions joint together by the common denominator of sudden bilateral ureteral obstruction, difficult to be suspected at times. Conversely, typical and well-known diseases h… Show more

“…13 Other causes also noted in our study were urethral strictures, prostate cancer, posterior urethral valve, ureteric ligation from total abdominal hysterectomy and pelvi-ureteric junction obstruction, as reported in the previous studies. 10,12,14,15 We also found a case of urethral stenosis in a female child similar to that reported from Port Harcourt. 16 Urinary diversion was done in 25% of our patients which is lower than 100% and 45% reported by the previous studies.…”

“…AGE-related urolithiasis can be found in otherwise healthy children, and its prognosis is generally favorable, although a subset of affected patients may require invasive procedures, such as nephrostomy. We believe that this rare complication of pediatric AGE is an underrecognized disease entity outside Japan, warranting further studies [5]. In clinical practice, urolithiasis should be included in the differential diagnosis for patients with AGE who present with symptoms, such as hematuria or persistent oliguria.…”

“…Improvement of kidney function is inversely proportional to the duration of obstruction. 5,12 In this study the duration of symptoms used as a parameter as the time of suspected obstruction to get treatment. In patients with release of obstruction procedure being performed ≤ 6 weeks after symptoms appeared, a significant and better reduction in serum creatinine compared with the release of obstruction performed > 6 weeks after the onset of symptoms.…”
