Acute Kidney Injury Masquerading as Acute Pancreatitis - Role of Serum Amylase and Lipase in Patients with Blunt Abdominal Trauma

Background: A typical manifestation of pancreatic tail cancer is large intestinal obstruction with perforation. Clinically the cancer of pancreas is usually complicated to diagnose. Most cancer patients are not having symptoms throughout during the initial stages of the cancer, which often leads to a delay in diagnosis. Treatment choices include surgery, chemotherapy, and palliative care. It is more common in African-Americans, slightly more common in men. Case Presentation: A female patient of 40 years from Wardha was admitted to Female surgery Ward, Unit-3, AVBRH on 18th December with a chief complaint of pain in epigastric region since 2weeks. Patient was apparently all right 2 months back then she was complaining of pain in the epigastric region which was insidious in onset, gradually progressive in nature, burning type of pain with radiating to left upper back. No history of fever, nausea vomiting, clay-coloured stools. After that patient was undergone on routine investigation in that total WBC count was increased i.e., 13000/cu mm and haemoglobin level were decreased i.e., 9.7gm%, liver biopsy revealed that metastasis of Adenocarcinoma probably of pancreatic origin, Computed tomography and ultrasound and it revealed that heterogenous iso-echoic mass in the tail of pancreas based on investigation she was diagnosed as a case of Carcinoma tail of pancreas and she was undergone on treatment of antibiotic before chemotherapy .after that chemotherapy treatment was done for management of pain. Conclusion: Pancreatic adenocarcinoma presents differently from large intestinal cancer and should be explored in the differential diagnosis of large intestinal obstruction.

Background: Congenital abnormalities of the kidneys and urinary tracts (CAKUT) are conditions brought on by errors in the growth of the kidneys and the outflow tubes that drain the kidneys. Although the issue may not become apparent until later in life, CAKUT is a congenital abnormality brought on by improper urinary system development. Case presentation: We are presenting the case of a 23-year-old man who has chronic kidney disease and is undergoing maintenance hemodialysis. The patient was apparently alright 2 weeks back when he had complained of breathlessness. fever 2 episodes, lower which was insidious in onset and gradually progressive. On arrival, the patient was provided oxygen therapy via a face mask for breathlessness and to make him feel comfortable with the head elevation. On laboratory investigation, the patient’s blood values are altered. The patient underwent computed tomography of the kidney-ureter-bladder (CT-KUB), and Ultrasonography of the kidneys (USG). A permanent tunnel hemodialysis catheter under local anesthesia was placed and he received 2 cycles of hemodialysis. During the course in the hospital, the patient’s condition was improving and was stable clinically and vitally hence, the patient was discharged. Conclusion: CAKUT can occur alone or in conjunction with heart and cardiovascular malformations, gastrointestinal tract malformations, central nervous system malformations, skeletal system malformations, lung, face, genito-reproductive system malformations, abdominal wall malformations, chromosomal abnormalities, MCA, and other organ system malformations.

Background: Acute pancreatitis is one of the most common abdominal pathologies having variable outcome ranging from self limiting abdominal pain to high mortality and morbidity due to organ failure, over the past five decades, various classification systems have emerged to classify pancreatitis according to its severity , and the associated complications have emerged to classify acute pancreatitis and its various complications, such as Apache Scoring , Ct Severity , Modified Ct Severity ,Ransen and Atlanta Classification. Objectives: We in our study will be classifying patients suffering from acute pancreatitis according to the revised Atlanta classification to divide them into interstitial edematous pancreatitis and necrotizing pancreatitis. The local complications will be classified according to CT imaging findings into acute necrotic collection, psuedocyst, acute necrotic collection and walled of necrosis. Organ failure will be assessed according to modified marshal scoring system into transient or persistent organ failure. Methodology: We will be carrying forward our study on Siemens 16 slice computer tomography machine over a sample of 140 patients coming to the outpatient department of our hospital which will be followed by a routine clinical follow up of the patient to find out their prognosis. Results: Will be tabulated at the end of the study using SPSS version 26 software. Conclusion: The revised Atlanta classification for acute pancreatitis, in conjunction with the Modified Marshall Scoring System for organ failure, if found useful, in our series in improving the prognosis of the patients, then it can be incorporated in management.
