Acute myocardial infarction in patients with normal coronary arteries after acute ethanol intoxication

Abstract: Summary:Three cases are presented where acute myocardial infarction occurred in young individuals after an episode of heavy alcohol intake. Subsequent coronary arteriograms demonstrated normal coronary arteries. Several mechanisms by which acute ethanol intoxication might precipitate myocardial infarction are discussed. To our knowledge, no similar cases have been reported.

“…MPV was measured in a blood sample collected in dipotassium EDTA tubes with an automatic blood counter (Beckman Coulter). For further determination of glucose, creatinine and lipid profi les, the second blood sample was drawn after a fasting period coronary trauma, coronary vasospasm and coronary thrombosis [5][6][7][8][9][10][11][12]. However, when specifi c aetiologies such as these are sought systematically, aetiology of MINC can be detected in only one-third of cases [22].…”

“…Various mechanisms have been hypothesized in myocardial infarction with normal coronary arteries (MINC), including transient occlusion of the infarct-related artery owing to platelet hyperactivity and thrombosis [5][6][7], coronary artery spasm [8], toxic conditions [9][10][11] and coronary embolization [12].…”

“…AMI has been recognized as a complication of cocaine abuse among young men and ethanol overdose may cause coronary vasospasm and death [33][34][35][36]. Cocaine use and alcohol use, both related to risk for injury [37][38][39], were entered as categorical variables.…”

“…Two patients had positive results after repeated tests. These findings suggest that specific provocative situations, such as high sympathetic activity, 12 heavy alcohol drinking 13 or local inflammation 14 could cause acute chest pain in patients with no previous history of variant angina or with a history of variant angina with low clinical activity. Another explanation could be the residual effect of vasodilatory drugs administered during the acute stages of MI.…”
