Acute Scrotum in Children

Abstract: Overlap existed between testicular torsion and other acute scrotum. The clinical manifestations, physical examination findings, laboratory, and radiographic data were helpful in distinguishing acute scrotum. Doppler ultrasound is an indispensable imaging modality for the clinical assessment of patients with acute scrotum; in the presence of a clinical suspicion of testicular torsion, even with an apparently normal-color Doppler ultrasound, surgical exploration is still indicated.

“…Various studies have shown that the leukocyte count is increased in patients with the diagnosis of testis torsion; it serves as an indicator of the inflammatory response. 6 A study in rats showed that testis torsion leads to an increased leukocyte count within the muscular tissue of the cremaster muscle, which surrounds the testicular tissue; moreover, that study showed deteriorating hypoxia, reduced microperfusion, and reduced capillary vessel function. 7…”

“…Modem ultrasound examination of the scrotum is an indispensable imaging modality for the clinical assessment of patients with acute scrotal pathology and yields high accuracy compared with surgical exploration. A key component of the testicular examination is use of power and spectral Doppler ultrasonography [32]. We should mention that the greatest limitation of our study was the incapablity of utilizing Doppler ultrasonography for all of the patients during the approach to diagnosis.…”

“…In children, an acute scrotal pain and swelling is a common reason for surgical consultation in the emergency department [ 1 ], and usually requires an exploratory surgery despite sonographic imaging findings [ 2 ]. Etiology of an acute scrotum has been reported to be torsion of the appendix in 70%, testicular torsion in 12% and epididymoorchitis in 11% [ 3 ].…”

“…Acute scrotal swelling with pain in children is a common entity that requires accurate evaluation followed by an appropriate management. Although most causes of an acute scrotum are non-emergent, some presentations may result in rapid loss of a testis if not diagnosed and managed in a timely measure [ 1 ]. The omental incarceration in an inguinal hernia in children is exceptional and very often remains an operative diagnosis despite progress of imaging in diagnosis of testicular pathology.…”
