Acute thrombosis after endarterectomy of stented left anterior descending artery

Abstract: Acute thrombosis of the endarterectomized coronary artery is a serious complication after coronary endarterectomy. Herein, we describe a case of a 65-year-old man who had undergone percutaneous stent implantation in the left anterior descending artery, then after he received an endarterectomy with the removal of stents severe in-stent restenosis occurred. Three days after the operation, sick sinus syndrome developed with acute myocardial infarction. Coronary angiography revealed thrombosis at the reconstructed… Show more

“…Our practice is to maintain dual antiplatelet therapy for at least three months following endarterectomy. In addition, the use of heparin and/or warfarin in the postoperative period has been described to avoid acute thrombosis 16,17 . We do not routinely employ anticoagulation for these patients.…”

“…The most disastrous complication after CE is acute thrombotic occlusion of the reconstructed LAD [24]. The mechanism of thrombus formation in the endarterectomized vessel is that the absence of endothelium exposes the subendothelial tissue to the bloodstream, which can trigger the coagulation cascade.…”

“…The most feared major complication following coronary stent endarterectomy in cases of in-stent restenosis is the occurrence of acute thrombosis in the stent endarterectomized artery [Uchimuro 2009]. Thrombosis occurs in these cases because of the loss of endothelium, exposing the subendothelial tissue to the blood stream, and consequently the coagulation cascade is triggered.…”
