Acute thrombus formation in the left atrium after the termination of warfarin

Abstract: We report a case of acute thrombosis formation in the left atrium 3 days after the discontinuation of warfarin therapy prior to mitral valve replacement in a patient with mitral stenosis and atrial fibrillation. A 58-year-old Asian female patient was scheduled for mitral valve replacement for mitral stenosis. She had received warfarin therapy every day for 2 years. “…Coagulation may surpass anticoagulation when the effect of anticoagulation is on the wane. A rebound thrombosis after discontinuing anticoagulant therapy was reported by Kadoi et al 13 Therefore, maintaining sufficient anticoagulation may be crucial following thrombolytic therapy for LA thrombus.…”

“…Before the initiation of thrombolytic therapy, echo characteristics of the thrombus must be carefully analyzed and a fresh, unorganized thrombus appears as homogenous, soft and mobile. A rebound thrombosis after discontinuing anticoagulant therapy was reported by Kadoi et al [81]. Therefore, maintaining sufficient anticoagulation was crucial following thrombolytic therapy for LA thrombus.…”
