Additive effects of nicorandil on coronary blood flow during continuous administration of nitroglycerin

Abstract: These results suggest that in patients treated with nitrates, additional administration of nicorandil is more useful, in terms of increasing CBF, than additional administration of nitrates. Adjunctive use of nicorandil with nitrates may provide the further benefit of myocardial protection and may improve the prognosis of patients with IHD.

“…It has been reported that the increase in coronary blood flow induced by bolus injection of nicorandil is only transient, and that coronary blood flow peaks at 1-2 min and returns to baseline 5 min after administration. 11 Because the decrease in IMR induced by nicorandil lasted for at least 10 min, improvement of abnormal coronary microcirculation may be prolonged once nicorandil normalizes the coronary microcirculation. Patients with high IMR following PCI should receive additional pharmacotherapy with agents such as nicorandil to decrease IMR.…”

“…Numerous researchers have reported the effects of nicorandil on CVD. For example, nicorandil improves coronary circulation without changing the systemic blood pressure in humans (6). Chronic administration of nicorandil to angina patients reduced cardiovascular events (CVE), such as cardiac death and nonfatal myocardial infarction (7).…”

“…25 Okamura et al have reported that nicorandil is more effective in increasing anterograde coronary blood flow than nitroglycerin alone, without causing the coronary steal phenomenon in patients with ischemic heart disease. 26 A recent report noted that nicorandil improves microvascular dysfunction (as assessed by the index of microcirculatory resistance) after primary percutaneous coronary intervention. 27 Nicorandil also exhibits cardioprotective effects in the ischemic myocardium, similar to those of ischemic preconditioning, via activation of mitochondrial KATP channels.…”
