Adenomyoepithelioma of the breast with malignant features

Abstract: The clinico-pathological features of 7 cases of adenomyoepithelioma of the breast with features suggestive of malignancy are presented. There was a high incidence of local tumour recurrence, in 2 cases as high-grade infiltrating carcinoma of the breast of no special type ("ductal", grade III). One patient died as the result of a clinically diagnosed cerebral metastasis. Histological examination of the primary breast tumours reveals two main patterns: (1) tumours consisting in part of typical adenomyoepitheliom… Show more

“…Presence of mitotic activity, necrosis, and cellular pleomorphism, and invasion at the periphery of the tumor indicate potential for locally aggressive behavior and/or metastasis which has been described in adenomyoepitheliomas of breast [18,19]. The present case did not exhibit any of these high risk features.…”

“…7,8 In some cases, mammography, sonography, and frozen sectioning of AME may suggest malignancy, which has major implications in the differential diagnosis and treatment of the lesion. 6 To our knowledge, this is the first report of the color Doppler sonographic findings of AME. A B…”

“…Although it is considered benign by nature, local recurrences and nodal and distant metastases in a bicellular pattern histologically similar to that of the primary tumor have been described. 1,[3][4][5][6]10,11 Recurrences have been attributed to incomplete resection because of the multinodularity and the peripheral intraductal extensions of the tumor. 1,11 Findings of high mitotic activity and cytologic atypia in both epithelial and myoepithelial cells may help in the differentiation of malignant and benign cases of AME.…”

“…45,[63][64][65][66] Pure myoepithelial tumors have only rarely been reported in the breast 3,[67][68][69][70][71] and account for about 3% of all tumors of salivary glands. [72][73][74][75][76] The differential diagnosis include spindle cell tumors with glandular 72,[77][78][79] and/or squamous differentiation, 67 spindle cell adenocarcinomas, [80][81][82][83][84] and mesenchymal tumors. 37 Here, we have only studied one malignant myoepithelioma of the breast, which expressed K5/K14 and the myoid marker SMA, but not, or only focally and rather feebly, glandular keratins K7/K8/K18 and/or keratin K10.…”
