L'effet de l'auto-plasma riche en plaquettes sur la régénération de l'anastomose intestinale chez le lapin Introduction. La régénération de la plaie est un processus important dans les pratiques chirurgicales. Cependant, l'échec d'une anastomose intestinale reste un problème de chirurgie intestinale. L'objectif de l'étude. Évaluer l'effet du plasma riche en plaquettes (PRP) et comparer l'efficacité de différentes méthodes d'application dans la régénération de l'anastomose intestinale chez le lapin. Méthodes. Un auto-plasma riche en plaquettes a été obtenu chez 81 lapins, qui ont été utilisés pour étudier deux méthodes d'application de PRP (injection dans la couche musculaire / immersion dans la paroi intestinale) par rapport au groupe témoin sur l'anastomose de bout en bout de l'intestin grêle.. Résultats. L'analyse des données histologiques et morphométriques de l'étude a montré que la régénération entre les échantillons d'anastomose intestinale traités avec du trempage au PRP et les injections dans la ABSTRACT Introduction. To evaluate the effect of platelet-rich plasma (PRP) and compare the effectiveness of various application methods in intestinal anastomosis regeneration in rabbits. Methods. Platelet-rich auto-plasma was obtained from 81 rabbits, which was used to study two methods of applying PRP (injection into the muscle layer / soaking in the intestinal wall) in comparison with the control group, on the end-to-end small-intestinal anastomosis. Results. The analysis of the histological and morphometric data showed that the regeneration between the samples of intestinal anastomosis treated with PRP soaking and injections into the intestinal wall is more favorable in comparison with the control group, which, in turn, was determined by the quantitative ratio of inflammatory infiltrate, fibroblast proliferation, neo-angiogenesis, and collagen deposition. Purpose of the Study: To analyze the effectiveness of using sodium deoxyribonucleate (SDN) for pelvic adhesions in rats in experiment. Materials and Methods: The experiment was carried out on 90 laboratory rats. Three groups of 30 animals each were formed: group 1 – control; group 2 – rats with a simulated adhesive process in the pelvis without subsequent treatment; group 3 – animals with a model of adhesive process in the pelvis receiving intramuscular SDN for 10 days. The adhesive process in the pelvis was modeled in rats by ligating the uterine horns on both sides. The expression of CD4, CD8, CD20, CD138 was studied using DP-SOFT software program and subsequent statistical processing. Results: In animals of the control group, a small number of cells with expression of CD4+, CD8+ and CD138 markers were found in the parietal peritoneum (PP). In perimetrium (PM), no positively stained cells of the studied immunophenotype were found. In group 2, animals showed an increase in the number of cells in the PP positively expressing CD4+ and CD138+, and the appearance of CD56+ cells. An increase in the number of CD138+ and CD4+ cells was found in the rats’ PM. In group 3, compared to the control, the count of CD4+ cells, CD56+ cells and CD138+ cells significantly increased. In the PM of the operated animals, we found an increase in the number of cells positively expressing CD4+ and CD138+, and the appearance of CD56+ cells. In the PM of rats, an increase in the number of CD138+ and CD4+ cells was noted, which indicates activation of the humoral immune system. In group 3, the count of CD4+, CD56+ and CD138+ – cells in the PP significantly increased compared to the control. PM was characterized by a similar distribution of cells. Conclusion: After surgery, the adhesive process causes a change in the ratio of leukocyte subpopulations in the affected area towards an increase in their number in the parietal peritoneum rather than in the perimetrium. The use of SDN has an immunomodulatory effect and stimulates immune responses by increasing the number of immunocompetent cells in the surgical site.

Aim: to assess the efficacy of penicillamine for the complex prevention of pelvic peritoneal adhesions in appendicitis associated with the acute conditions of uterine appendages (appendicular-genital syndrome, AGS) based on the changes in the levels of hydroxyproline and the major clinical symptoms of pelvic inflammatory disease. Patients and Methods: 157 women of reproductive age with AGS who were urgently or routinely admitted to a gynecological or surgical hospital were examined. Comparison group women were prescribed with conventional anti-inflammatory treatment or surgery as needed. Study group women additionally received penicillamine 250 mg twice daily for a month. Pre- and post-treatment non-invasive assessment of the peritoneal adhesive process was performed by measuring the levels of protein-bound and free hydroxyproline. Treatment efficacy was evaluated after 1 month by the changes in hydroxyproline levels, major clinical symptoms, and ultrasound. Results: it was demonstrated that pelvic acute inflammatory disease results in > 1.5-fold increase in the levels of protein-bound and free hydroxyproline. Penicillamine as a component of the complex treatment for pelvic inflammatory disease reduces the levels of hydroxyproline to normal ranges. The changes in the major clinical symptoms and ultrasonic findings after penicillamine therapy were more significant compared to the standard treatment. Conclusions: penicillamine as a component of the complex treatment for pelvic inflammatory disease significantly accelerates the improvement of symptoms and remission as well as the formation of peritoneal adhesions. This prevents both pelvic adhesive disease and tubal- and peritoneal-factor infertility.
