Adipocytes and Neutrophils Give a Helping Hand to Pancreatic Cancers

Abstract: Summary: Obesity-induced inflammation can build up a confined microenvironment in pancreatic adenocarcinoma that is associated with increased desmoplasia, neutrophil recruitment, reduced delivery of chemotherapeutic drugs, and immune evasion. Targeting molecular pathways empowering this circuit might represent a necessary measure to reach clinical efficacy for combination therapies in patients with excess body weight. “…40,41,45 This systemic and local inflammatory milieu may be conducive to tumor initiation and/or promotion. 60,61 The inflammatory microenvironment also is thought to be the major mechanism, by which chronic pancreatitis leads to PDAC development. 6265 Targeting pancreatic inflammation by inhibiting cyclooxygenase activity, using aspirin, or targeted blockade of inflammatory cytokines has been shown to attenuate cancer development and/or growth.…”

“…38 In addition, neutrophils have potential relevance to tumor progression, because emerging evidence suggests that neutrophils are active factors in cancer progression 39 and in the suppression of the antitumor immune response. 40 Thus, it is conceivable that tumor-adipocyte crosstalk modifies the inflammatory status of the tumor microenvironment, inducing tumor progression. In support of this model, SAA1-overexpressing TNBCs experienced greater infiltration by neutrophils and other pro-tumor immune cells than SAA1-low tumors.…”

“…Neutrophil recruitment appears to be the primary event in the initiation of the inflammatory cascade in adipose tissue during metabolic disorders 37 and mediates the development of AT inflammation 38 . In addition, neutrophils have potential relevance to tumor progression, because emerging evidence suggests that neutrophils are active factors in cancer progression 39 and in the suppression of the antitumor immune response 40 . Thus, it is conceivable that tumor‐adipocyte crosstalk modifies the inflammatory status of the tumor microenvironment, inducing tumor progression.…”

“…On the other hand, the TME can provide an immunosuppressive niche to negatively regulate immune effector cells and facilitate the malignant progression of cancer (Ho et al, 2020), and causes the resistance of PC against various treatments such as chemotherapy, targeted therapy, and immunotherapy (Balachandran et al, 2019). The major immunosuppressive cell types in the TME of pancreatic cancer are tumor-associated macrophages (TAMs), myeloidderived suppressor cells (MDSCs), regulatory T cells (Treg), TH17, and tolerogenic DCs (Bronte and Tortora, 2016;Zhu et al, 2017;Ligorio et al, 2019;Zhang et al, 2020). These immunosuppressive cells promote tumor progression through a variety of mechanisms, including the direct mediation, inhibition of tumor-killing immune cells, induction of angiogenesis and lymphoangiogenesis (Ho et al, 2020), and promotion of metastasis.…”
