Adjunctive subantimicrobial dose doxycycline: effect on clinical parameters and gingival crevicular fluid transforming growth factor‐β1 levels in severe, generalized chronic periodontitis

Abstract: These data indicate that combination of SDD with non-surgical therapy improves clinical parameters of periodontal disease and increases GCF TGF-beta1 levels together with a decrease in prevalence of residual pockets in patients with severe, generalized chronic periodontitis. Increased GCF TGF-beta1 levels following SDD therapy might suggest a novell pleiotrophic mechanism for tetracyclines to inhibit connective tissue breakdown.

“…This finding is in agreement with previous reports which found lower GCF TGF-β1 concentrations at inflamed sites when compared with healthy sites (Buduneli et al, 2001, Gürkan et al, 2005Kuru et al, 2004a). Expression of GCF constituents as concentration could result in higher levels at healthy sites where GCF volume is very low (Curtis et al, 1988, Emingil et al, 2000.…”

“…The authors concluded that the drug efficacy at the biochemical level may continue as long as the agent is used. In accordance with our results, Gürkan et al (2005), have demonstrated that the level of this growth factor increased after resolution of inflammation. Thus, it could be hypothesized that antiinflammatory role of TGF-β1 may be more potent than its proinflammatory properties during healing after NPT.…”

“…Studies have reported the anti-periodontitis activity of films or other localised oral mucosal or gingival drug delivery systems [18][19][20][21][22][23]. Many studies have reported on the potential of novel ciprofloxacin delivery devices in the treatment of periodontitis [24][25][26][27].…”

“…Furthermore, low-dose doxycycline seems to be able to normalize lowered levels of cytokines necessary for repair and regeneration. In patients with severe generalized chronic periodontitis, subantimicrobialdose doxycycline increases the levels of TGF-␤ 1 in the gingival crevicular fluid and by this means contributes to the inhibition of connective tissue breakdown [72] . For rosacea no corresponding studies are available so far, but it is to be assumed that the described mechanisms are, at least partly, transferable to this disease as well.…”
