Adverse Drug Reaction reports for cardiometabolic drugs from sub‐Saharan Africa: a study in VigiBase

Abstract: Abstractobjective Identifying key features in individual case safety reports (ICSR) of suspected adverse drug reactions (ADRs) with cardiometabolic drugs from sub-Saharan Africa (SSA) compared with reports from the rest of the world (RoW). . We used vigiPoint, a logarithmic odds ratios (log 2 OR)-based method to study disproportional reporting between SSA and RoW. Case-defining features were considered relevant if the lower limit of the 99% CI > 0.5.

“…For established products, data are primarily derived from high-income countries with well-established PV systems [7][8][9]. This is also problematic because the safety profile of certain products may differ between settings due to a variety of factors, including environmental and genetic influences [10][11][12]. It is therefore important that appropriate safety monitoring practices are in place in SSA for both established and newly launched products.…”

“…Our study suggests that the harmful aspects of medication use receive little attention, even in research. In line with this, adverse drug reaction reporting from sub-Saharan Africa is the lowest in the world [93,94].…”

“…Fourthly, late adverse drug reaction to the ACEIs can occur up to thirteen years of medicine use. Frequent side effects associated with the use of ACEIs family of anti-hypertensive medicines are intractable non-productive cough and the conspicuous labial-facial angio-edema or Quincke’s edema [3, 7]. When any of these occur, the drug and related medicines are immediately stopped and black listed for the patient.…”

“…Also, a 74 year old man had to take over thirty medicines including steroid injections daily, cumulated from various consultations from many physicians until he developed iatrogenic Cushing syndrome and multiple organ damage. When switching to another medicine in the elderly, we should ensure that the old and/or frequently used medicines are discarded to reduce the risk of drug intoxication or drug-drug interaction [3]. Also, we should limit our prescription to the most needed medicines, and choose from the once daily intake or the most simple drug regimens [10].…”
