A 49-year-old woman developed an anaphylactic reaction to Tc-99m macrosalb used for pulmonary scintigraphy. The patient received an intravenous injection of Tc-99m macrosalb 120 MBq, containing macroaggregates of human albumin 0.14 mg. Within 1 min she developed itching all over her body, an itching throat and dyspnoea. This was followed by urticaria and facial oedema. She was diagnosed with an anaphylactic shock. The patient received clemastine and prednisone, and fully recovered after release from the hospital. According to the Naranjo assessment algorithm, the relationship between the allergic reaction and the administration of Tc-99m macrosalb should be considered as ‘probable’.

Hypocalcemia is one of manifestation of bone metastases which could be treated using calcium carbonate and cholecalciferol. Tc-99 m radiolabeled 1,4,8,11tetraazacyclo tetradecyl-1,4,8,11-tetramethylene phosponic acid (against 99m Tc-CTMP) on the other hand is a radioactive complex compound which has an affinity toward bone. Therefore, it could be used as bone tracer (radiopharmaceutical) in bone imaging. However, there has been a concern regarding the use 99m Tc-CTMP on patients who have been treated with calcium carbonate and cholecalciferol. In this circumstance 99m Tc-CTMP could interact with calcium carbonate and cholecalciferol and it might then interefere with the imaging results. Therefore, the aim of this study was to determine the pharmacokinetic behavior of 99m Tc-CTMP in rats (Rattus norvegicus) that had been previously treated with calcium carbonate and cholecalciferol. The pharmacokinetic studies were performed using four groups of animal model and each groups consists of three rats. The groups were classified as control (I), rats treated with calcium carbonate (II), rats treated with cholecalciferol (III), and rats treated with both calcium carbonate and cholecalciferol (IV). After the rats were treated for 3 days, 99m Tc-CTMP radiopharmaceutical was injected through a tail vein of each rat. At specific time intervals after 99m Tc-CTMP administration, blood was then extracted from the tail, weighed and counted using a single channel analyzer. The percentage of radioactivity in blood at a certain interval was then calculated to determine the distribution half-time and the elimination half-time. The distribution half-time of group I, II, II, and IV were 0.43+0.13, 0.25+0.18, 0.32+0.10, and 0.47+0.07 hours, respectively, while the elimination half-time were 2.56+0.18, 4.48+0.56, 4.47+1.13, 6.19+1.97 respectively. The results of T-test showed that there was no significant difference of distribution half-time between the three treated groups and the control group. However, there was significant difference of elimination half-time between the three treatment groups and control. This research showed that giving calcium carbonate or, cholecalciferol, and both for 3 days prior administration of 99m Tc-CTMP would mantain elimination half-time which resulted in longer excretion/ elimination time.

Zusammenfassung Ziel Die neue Strahlenschutz-Gesetzgebung sieht vor, dass eine Risikoanalyse neuer und bestehender Behandlungsverfahren mit offenen Radionukliden hinsichtlich der Patientensicherheit wiederkehrend durchzuführen ist, in welche der Medizinphysik-Experte maßgeblich involviert ist. Material und Methoden Als Werkzeug für die Durchführung der Risikoanalyse wurde die Fehlermöglichkeits- und Einflussanalyse (FMEA) angewendet, wie vom Bundesamt für Strahlenschutz (BfS) und den Fachgesellschaften empfohlen. Für die Radiojodtherapie, die Radiosynoviorthese, die Peptid-Radiorezeptortherapie und die selektive interne Radiotherapie wurden die Prozesse gegliedert und zunächst individuell durch die verschiedenen am Prozess beteiligten Berufsgruppen und anschließend im Konsens kategorisiert. Für Risikoprioritätszahlen > 125 wurden Gegenmaßnahmen erarbeitet. Die Risikoprioritätszahl (RPZ) ergibt sich als Produkt aus dem Schweregrad, der Auftretenswahrscheinlichkeit und der Entdeckungswahrscheinlichkeit des Fehlers oder der Folgen. Ergebnisse Neben den diversen Möglichkeiten der Verwechslung von Patienten wurden Unsicherheiten in der Aktivitäts- und Organgrößenbestimmung und eine mangelnde Compliance des Patienten als wesentliche Risiken für eine Unter- oder Übertherapie angesehen. Die ausgeprägte Streuung bei individueller Risikobetrachtung durch einzelne Berufsgruppen konnte unter Anleitung durch einen externen Moderator deutlich reduziert werden. Für die höchsten RPZ wurden Maßnahmen formuliert, deren präventive Wirkung im Verlauf noch zu überprüfen ist. Schlussfolgerung Die nuklearmedizinische Therapie wurde in der organisatorischen Durchführung als sehr sicher eingeschätzt und es wurden nur geringe Risiken bzgl. der Patientensicherheit identifiziert. Die FMEA-Analyse war ein praktikables Instrument und identifizierte Prozessschritte mit Optimierungspotenzial für die analysierten Therapieformen. Das geschilderte Vorgehen kann in anderen nuklearmedizinischen Einrichtungen adaptiert werden, wobei strukturspezifische Faktoren (technische und personelle Ausstattung, Ablauf von Prozeduren) zu berücksichtigen sind, sodass ggf. weitere Risiken identifiziert und hier aufgeführte Risiken aufgrund anderer Abläufe als abweichend eingeschätzt werden können.
