Age‐Specific Prevalence of Anal Human Papillomavirus Infection in HIV‐Negative Sexually Active Men Who Have Sex with Men: The EXPLORE Study

Abstract: Urban, HIV-negative MSM have a stable, high prevalence of anal HPV infection across all age groups. These results differ substantially from the epidemiologic profile of cervical HPV infection in women. This may reflect differences between these populations with respect to the number of new sex partners after the age of 30 and may explain the high incidence of anal cancer in MSM.

“…The greater cancer prevalence among gay men may be caused by a higher rate of anal cancer, as suggested by earlier studies that point to an excess risk of anal cancer among gay men, [16][17][18] including an ecological study that linked high incidence rates of anal cancer to the high proportion of gay men living in San Francisco. 19 We did not have data available on the rate of human immunodeficiency virus (HIV) infection, which is higher among gay men, and may have contributed to the significant association of cancer prevalence and sexual orientation.…”

“…Our data are restricted to participants who were asked about their sexual orientation; these were respondents aged 18 Demographic characteristics were ascertained from all individuals, including sex, race, current age, educational attainment, income (defined as total annual household income), health insurance status, and nativity, distinguishing US-born from foreign-born respondents. General health distinguished self-reports of health as excellent, very good, or good from self-reports of health as fair or poor.…”

“…Among HIV-negative MSM, HPV prevalence is around 50% to 60% and remains at a plateau high throughout adulthood, well into the sixth decade of life. 17 Presumably, the rate in HIV-positive men during the life span is even higher.…”

“…Moreover, reductions in smoking among MSM would mitigate risks for health problems that affect them disproportionately; for example, smoking appears to be a potent yet modifiable cofactor in the development of anal cancer. [14][15][16][17][18] Targeting smoking prevention and cessation initiatives among MSM is difficult since only a handful of studies have investigated correlates of smoking among sexual minorities. 3,4,6,11,13 We undertook the present study to determine the prevalence and correlates of smoking in a community-recruited cohort of urban-dwelling Canadian MSM.…”
