AIDS-related non-Hodgkin's lymphoma presenting as delayed healing of an extraction wound

Abstract: Non-Hodgkin's lymphoma (NHL) of the oral cavity frequently occurs in patients infected with human immunodeficiency virus (HIV). This report describes a lesion presenting as delayed healing of an extraction wound with hyperaemic swollen gingivae and ulceration in an apparently healthy 34-year-old Thai fisherman. The lesion was the first evidence of his HIV-positivity. It is, therefore, imperative that clinicians should consider a diagnosis of HIV infection in cases of non-healing extraction wounds in patients i… Show more

“…The oral lesions are characterised by swellings which grow rapidly and may ulcerate. The most common oral sites are the gingivae and palate, but cases of lymphoma presenting as delayed healing of extraction sockets have been noted (Nittayananta et al, 1996). Early recognition, biopsy and appropriate referral are essential in the management of oral lesions.…”

“…Of interest, the lesions were multifocal and initially presented as a periodontal disease. Changes in periodontal tissue such as gingival swelling, necrotising ulcerative gingivitis and alveolar bone loss, have been previously reported as a clinical manifestation of HIV-related NHL (Vallejo et af, 1989;Nittayananta et al, 1996). The differential diagnosis included periodontal disease, deep fungal infections, granulomatous diseases, and malignant neoplasms.…”

“…However, the incidence of malignancy in patients who are immunodeficient has been reported as 10,000 times that of the general age matched population. [1112] The incidence of diffuse large cell lymphomas is 0.3% in younger age and has a relative survival rate of 42.9% after 5 years. [13] Primary immunodeficiency syndromes such as Chediak-Higashi, Wiscott-Aldrich, and ataxia-telangiectasia are associated with an increased risk of developing lymphoma.…”

“…NHL in patients with AIDS confers a poor prognosis, not only because of the sequelae of progressive lymphoma, but also because of an increased incidence of opportunistic infection. [11] The overall prognosis was significantly worse among HIV-positive patients. Median survival was 34 months in immunocompetent vs .…”
