All-terrain vehicle injuries in children: injury patterns and prognostic implications

Abstract: A wide variety of injuries were identified in children with ATV accidents. Partial foot amputation, an unusual injury, was identified in three children. Brain injuries and lung contusions detected by computed tomography were associated with long-term disability and death. Radiologists need to be aware of injuries associated with ATV accidents.

“…The CPSC reported that 50% of all ATV accidents involve a rollover event (16). Fourteen studies have reported musculoskeletal injuries from ATV accidents in the United States (8,(11)(12)(13)(14)(15)(16)29,(35)(36)(37)(38)(39)(40). Only six reports detailed the specific accident mechanism (12,14,19,(25)(26)(27).…”

“…In a retrospective study of radiographic data from the University of Arkansas, Bhutta et al reported that 53% of patients who were involved in an ATV accident sustained some type of fracture (40). Although only a relatively small percentage of individuals who sustained an injury from an ATV accident required hospital admission, Rodgers and Adler reported that the injuries observed in this patient subset were often quite serious (36).…”

“…8 Brain injuries are among the leading cause of death and disability from ATV crashes. [23][24][25] They are more likely to occur on the roadway than off road, even when controlling for helmet use. 8,17 This pattern may be due to forces in roadway crashes great enough to exceed the protective capacity of helmets.…”

“…15 Upper limb fractures are less common. 18 Although rarely reported, 9 vertebral fractures (as illustrated by Case 1 in the present series) cause significant morbidity and in the case of cervical fractures they are often incompatible with life. Thoracolumbar injury is most common 16 but cervical fractures have greater negative impact on survival.…”
