Allergic Contact Dermatitis

Abstract: Allergic contact dermatitis (ACD) is a common skin disease caused by a T cell-mediated immune reaction to usually innocuous allergens. ACD can have grave medical and socioeconomic consequences. ACD and irritant contact dermatitis often occur together. A detailed history and clinical examination are crucial and guide patch testing, which is the gold standard to diagnose ACD. T-cell clones persisting in the skin may explain the tendency of ACD to relapse even after years of allergen avoidance. Traditional treatm… Show more

“…An estimated 15–20% of the general population are affected by contact dermatitis, and regarding occupational diseases, irritant contact dermatitis and allergic contact dermatitis (ACD) are among the most common of these medical conditions . Although an impressive amount of research has identified two distinct pathophysiological phases in the development of ACD, namely sensitization and elicitation, the complex immune mechanisms behind them are incompletely understood …”

“…Unlike irritant contact dermatitis, which results from an irritant chemical causing a direct toxic effect on the skin, ACD is a delayed type IV hypersensitivity reaction to a hapten or antigen. 61 Unlike other hypersensitivity reactions, ACD is not dependent on antibodies but relies on a T H 1 response to an antigen presented by an antigen-presenting cell. It is dependent on 2 phases: the induction phase, whereby the immune system is sensitized to an antigen, and the elicitation phase, whereby the immune response is triggered on rechallenge of the allergen.…”

“…Allergic contact dermatitis (ACD) is the most common cause of occupationally related skin disease, affecting as many as 1 in 7 individuals annually (Kostner et al, 2017;Thyssen et al, 2009). Although the conceptual pathologic process in ACD is straightforward-an inappropriate or exaggerated immune response to skin contact with an otherwise innocuous substance-the precise immunologic mechanisms mediating this pathologic process are complex and incompletely understood.…”
