Alopecia in association with severe seborrhoeic dermatitis following combination antiretroviral therapy for acute retroviral syndrome

Abstract: We present a case where alopecia occurred with severe seborrhoeic dermatitis associated with the commencement of combination antiretroviral therapy for acute retroviral syndrome. We postulate that the eruption could represent a novel manifestation in association with immunological response to antiretroviral therapy.

“…Seborrhoeic dermatitis (localized and not severe or atypical) was the commonest IRIS condition in our study but has only once been previously described as an example of IRIS, when it was coupled with alopecia and occurred 1 month after HAART being started for acute retroviral syndrome 8 . Seborrhoeic dermatitis has been reported in up to 83% of people infected with HIV, so its presence as an IRIS event could be expected.…”

“…Seborrhoeic dermatitis has been reported in up to 83% of people infected with HIV, so its presence as an IRIS event could be expected. Its aetiology is poorly understood, but there is an association with Malassesia furfur infection, against which the inflammatory component of IRIS may be directed 8 …”

“…The cross-sectional nature of the measurements of blood metals and personal behaviors (for example, sleep patterns [67], [68], [70], [94] and oily scalp [62], [95], [96], [97], [98], [99], [100]) limit our ability to identify causal relationships; the time of onset of moderate to severe hair loss will need more investigation in future work.…”

“…108 Response to antiretroviral treatment is variable and there have been conflicting reports in the literature: some investigators reported an improvement in SD with the initiation of antiretroviral treatment, while others reported aggravation. 109,110 Furthermore, there have been reports of aggravation during immune reconstitution inflammatory syndrome and reports that antiretroviral treatment does not alter the prevalence or the course of SD. 111,112 With respect to histopathology, the findings of SD in HIV-positive patients are similar to those found in HIV-negative patients; however, there appears to be greater follicular involvement in the lesions and more plasmocytes in the inflammatory infiltrate in the HIVpositive patients.…”
