Past studies in humans have demonstrated horizontal and vertical bone loss after six months following tooth extraction. Many biomaterials have been developed to preserve bone volume after tooth extraction. Type I collagen serves as an excellent delivery system for growth factors and promotes angiogenesis. Calcium phosphate ceramics have also been investigated because their mineral chemistry resembles human bone. The aim of this study was to compare the performance of a novel bioresorbable purified fibrillar collagen and hydroxyapatite/β-tricalcium phosphate (HA/β-TCP) ceramic composite versus collagen alone and a bovine xenograft-collagen composite in beagles. Collagen plugs, bovine graft-collagen composite and HA/β-TCP-collagen composite were implanted into the left and right first, second and third mandibular premolars, and the fourth molar was left empty for natural healing. In total, 20 male beagle dogs were used, and quantitative and histological analyses of the extraction ridge was done. The smallest width reduction was 19.09% ± 8.81% with the HA/β-TCP-collagen composite at Week 8, accompanied by new bone formation at Weeks 4 and 8. The HA/β-TCP-collagen composite performed well, as a new osteoconductive and biomimetic composite biomaterial, for socket bone preservation after tooth extraction.

Primary stability is an indicator of subsequent osseointegration of dental implants. However, few studies have compared the implant stability among anatomical regions and bone types; thus, not enough data exist regarding the stability of implants placed in regenerated bone (RB). The present study evaluated primary and long-term stability of implants placed in RB and non-regenerated healed bone (HB). A total of 216 screw cylinder implants were placed in 216 patients (98 in HB and 118 in RB, 6 [RB6, N = 68] or 12 [RB12, N = 50] months after tooth extraction). Implant stability was evaluated using resonance frequency analysis (RFA) measured at the time of implant placement (E1), at the time of loading (4 months after placement, E2), and 4 months after loading (E3). Various clinically relevant measurements were obtained, such as implant diameter, length, and location, as well as bone quality. At E1, implant location, bone quality, and experimental group significantly affected implant stability (all at P < .05). At E2, implant location, diameter, length, and experimental group significantly affected implant stability (all at P < .05). At E3, bone quality, implant diameter, length, and experimental group significantly affected implant stability (all at P < .01). Stability for the RB12 group was significantly higher than all other corresponding values; further, the values did not change significantly over time. For the HB and RB6 groups, stability was significantly higher at E2 than at E1 (P < .001) and was no different between E2 and E3. Implant location, length, and experimental group were associated with these differences (all at P < .05). Compared with HB and RB6, higher implant stability may be achieved in regenerated bone 12 months post-extraction (RB12). This stability was achieved at E1 and maintained for at least 8 months. Variables such as implant length, diameter, and bone quality affected the stability differently over time. Implant stability varied in different anatomic regions and with regard to different healing processes in the bone.

Aim: To Improve the treatment of patients with partial edentia and chronic generalized periodontitis by including dental implantation to the complex of therapeutic measures. Materials and methods: There have been presented the results of 240 patients with varying severity degrees of generalized periodontitis who underwent surgery on periodontal tissues with dental implantation. The research was conducted at the OCH of Ivano-Frankivsk 2007–2017.To improve the efficiency for dental implants and periodontal surgical intervention is necessary to determine the quality of bone remodeling, identifying markers of bone tissue metabolism. Results: It was established that in 40,81 % patients the results of research showed markers of bone remodeling indicating a high rate of bone formation (25,12±2,23 ng / ml) and a slight increase in rate of resorption marker, which results in low rates of bone resorption (8,54±1,23 nmol/L). These patients were not prescribed with osteotropic drugs. 50,83 % of patients with osteopenia were noticed to manifest the formation of bone possible raise of bone resorption (10.82±1.34n/mole), which was the indication for antiresorptive medication prescription. In 8,36 % of patients with osteoporosis was observed inhibition of bone formation processes (18,05±2,08 ng / ml) and increased resorption indices (15,34±1,87 nmol/L). Medications that stimulate osteogenesis and prevent bone resorption were prescribed. Conclusions: When planning dental implants and periodontal surgical intervention it is necessary to identify markers of bone remodeling to assess the structural and functional state of bone tissue and prescribe osteotrophic drugs that promote positive postoperative period.
