Background: Primary intraosseous carcinoma (PIOC), NOS is an odontogenic carcinoma with unknown etiology. Its diagnosis may be used when central jaw carcinoma cannot be categorized as any other type of carcinoma. Further information on this extremely rare tumor is needed to improve our understanding and evaluate the classification of odontogenic carcinomas. Case presentation: We herein presented two patients with PIOC, NOS with different clinical and histopathological features and analyzed gene mutations in these patients using next-generation sequencing (NGS). The typical PIOC, NOS case had many histopathological similarities to oral squamous cell carcinoma (OSCC), including the missense point mutations of TP53 Glu285Val, KDR Gln472His, and APC Pro1433Leu, which are similar to those in other cancers; however, no mutations were detected in the other patient with an atypical presentation of PIOC, NOS, which was derived from a precursor cystic lesion with similarities to both ameloblastic carcinoma and OSCC. Conclusions: Genetic analysis suggested that these two PIOC, NOS cases have different features and can be subcategorized.

Dentigerous cyst is the most common odontogenic cyst of the jaws after radicular cyst and is usually observed as unilateral involvement. The multiple involvement of this cyst is rather rare if not accompanied of an underlying systemic disease or syndrome. In this report, radiographic findings and surgical treatment of a patient with non-syndromic multiple dentigerous cyst, who applied to the clinic for prosthetic treatment were presented. Panoramic radiograph revealed radiolucencies associated with the crowns of bilateral mandibular third molar teeth, right maxillary third molar, bilateral maxillary canines. In cone-beam computed tomography (CBCT), it was revealed as well-defined unilocular radiolucent areas surrounded by sclerotic border and related to the crown of the unerupted teeth. In intermittent sessions, multiple dentigerous cysts were enucleated and the associated impacted teeth were extracted under local anesthesia. According to the comprehensive research, this is the first case-report presenting the surgical treatment and revealing the radiographic findings in elderly non-syndromic patient with multiple dentigerous cyst in maxilla and mandible.

Impaksi gigi merupakan kondisi gigi tidak dapat tumbuh ke rongga mulut, dan masuk ke lengkung rahang. Kista dentigerous adalah kista di sekitar mahkota gigi yang terbentuk pada gigi belum erupsi. Kista dentigerous pada gigi belum erupsi terbentuk apabila di dalam lapisan epitel email yang tereduksi menumpuk cairan. Kista ini dapat menumpuk di antara epitel dan mahkota gigi yang belum erupsi. Laporan kasus ini bertujuan untuk memperlihatkan perubahan dari lesi pre-kista yang tidak dilakukan perawatan menjadi kista dentigerous dalam jangka waktu dua tahun setelah pasien melakukan perawatan gigi rutin pertama. Kasus ini pada pasien perempuan, berumur 19 tahun dengan keluhan terdapat gigi yang tidak tumbuh pada rahang atas dan terasa berjejal. Pasien ingin dirapikan giginya untuk alasan estetika. Hasil pemeriksaan radiograf panoramik ditemukan perkembangan lesi secara tidak sengaja. Dari radiograf rutin panoramik, terlihat gigi impaksi dengan gambaran lesi prekista dentigerous pada bagian mahkotanya. Dua tahun setelah perawatan pembersihan karang gigi dilakukan radiograf oklusal, terlihat lesi tersebut membesar dan telah berubah menjadi kista dentigerous. Erupsi gigi yang terlambat atau adanya impaksi pada gigi dapat menyebabkan penumpukan cairan di dalam lapisan epitel email gigi. Kista dentigerous dapat terbentuk karena ada cairan yang menumpuk di antara epitel dan mahkota gigi. Pertumbuhan kista yang lambat dan tidak adanya gejala penyerta menyebabkan kista ini baru terdeteksi dari pemeriksaan radiograf rutin.
