An effective antibiotic cover for the prevention of endocarditis following dental and other post-operative bacteraemias

Abstract: SYNOPSIS Pre-and post-extraction blood cultures were taken from 242 patients. The post-extraction ones were taken from 100 unpremedicated patients, from 42 with an erythromycin estolate cover, and from 100 patients after protection with pyrrolidino methyl tetracycline. The 100 post-extraction blood cultures from unpremedicated patienits gave 64 positive results which yielded 155 strains, 88 of which were not aerobes. One hundred and fifteen representative strains were tested for sensitivity to 22 antibiotics. “…The present study indicates that the intensity of bacteremia was 2 cfu/mL or less in 80% of positive cases, which is comparable with the 75% of positive cases having one colony per mL of blood, as reported by Khairat (1966).…”

“…This may be due to difficulty in the culturing of small samples of blood or, as suggested by Robinson et al (1950), the bacteria are unevenly distributed, and a large blood sample is required for detection to be ensured. The results of the present study are comparable with rates detected in adults (Everett and Hirschmann, 1977;Khairat, 1966).…”

“…Another feature of the study was the large number of species of organisms isolated. Thirty different species were isolated including many anaerobes, this number being even greater than numbers of species recovered in studies assessing bacteraemia following tooth extractions (Khairat 1966, Sconyers et al 1973. The high number of anaerobes might be expected when one considers the predominantly anaerobic microflora of the gingivai sulcus and the particular brushing and culture techniques used.…”

“…5 IE is sometimes preceded by an interventional procedure, and antibiotics given before such procedures reduce the frequency of positive blood cultures. 6 Although their validity has been questioned, antibiotics reduce the frequency of episodes of IE in some animal models.…”
