Aerosol inhalation is considered the optimal route for administering the majority of drugs for the treatment of obstructive airways diseases. A number of Pressurised Metered-Dose and Dry Powder Inhalers are available for this purpose. However, inhalation of therapeutic aerosols is not without difficulty; it requires precise instructions on the inhalation manoeuvre, which is different from spontaneous normal breathing. Also, the characteristics of the inhaler device have to be suitable for the user. Available data indicate a frequent lack of knowledge demonstrated by health professionals and patients on the inhalation manoeuvre and handling of inhalers, resulting in a reduction of therapeutic benefit. This paper reviews the literature concerning the fundamental aspects of inhaler devices, inhalation manoeuvre and device selection, in an attempt to increase the knowledge of, and to optimise the clinical use of, therapeutic inhalers.

The bronchodilator effects of inhaled dry salbutamol powder (50, 100, 200 and 400 μg) have been compared in ten asthmatic patients with those of aerosolized salbutamol (200 μg) delivered from a standard pressurized inhaler. A dose of salbutamol powder (400 μg) produced greater bronchodilatation than 50, 100 or 200 μg of powder, but there was no significant difference between 400 μg of powder and 200 μg of aerosolized salbutamol. Powder administered by Rotahaler is well tolerated and offers several advantages over the pressurized aerosol canister.

Comparative Study of the Bronchospasmolytic Effect of Fenoterol (0.2 mg) and Salbutamol (0.4 mg) as Powder inhalations in 20 Patients with Reversible Bronchial Obstruction

In a double-blind crossover study, the effects of fenoterol powder inhaled from capsules (0.2 mg) and the powder inhaler with salbutamol capsules (0.4 mg) were compared. A single dose of each was administered on 2 subsequent days in random order. The effects on lung function test results were followed from 15 to 360 min after administration. Apart from the overall comparison, special attention was paid to the beginning and the end of the test periods. Statistically significant differences were found only in a few parameters and showed a trend for fenoterol to have a somewhat longer effect and for salbutamol to have a faster onset of effect. The differences between the effects of the two drugs were clinically not relevant. Blood pressure and heart rate were unaffected by either treatment.
