An overview of the evolution of enterovirus 71 and its clinical and public health significance

Abstract: Since its discovery in 1969, enterovirus 71 (EV71) has been recognised as a frequent cause of epidemics of hand-foot-and-mouth disease (HFMD) associated with severe neurological sequelae in a small proportion of cases. There has been a significant increase in EV71 epidemic activity throughout the Asia^Pacific region since 1997. Recent HFMD epidemics in this region have been associated with a severe form of brainstem encephalitis associated with pulmonary oedema and high case-fatality rates. The emergence of la… Show more

“…Permanent paralysis has only rarely been observed in humans. 37 Based upon the evidence, we conclude that EV71 infection in rhesus monkeys could promote pathological lesions in the CNS, which can be clearly determined according to a pathological index, although no specific clinical symptoms manifested.…”

“…During the course of the outbreak, more than 176,000 cases of infection were reported and at least 100 individuals died (Yang et al, 2009). HFMD has also been reported in Australia, the AsiaPacific region, and several European countries since its appearance (McMinn, 2002). During recent years, HFMD surveillance has shown increasing infections in the AsiaPacific region, especially in Malaysia, Singapore, Taiwan and mainland China.…”

“…In infection by poliovirus (PV), which is the causative agent of poliomyelitis and belongs to the species Human enterovirus C, motor neurons are the major target for neurovirulence (Bodian, 1949). Enterovirus 71 (EV71), another neurotropic enterovirus belonging to the species Human enterovirus A, is a causative agent of hand, foot and mouth disease and herpangina, but sometimes causes severe neurological diseases, such as brainstem encephalitis and poliomyelitis-like paralysis (Chumakov et al, 1979;McMinn, 2002;Wang et al, 2003). EV71 causes fatal pulmonary oedema and pulmonary haemorrhage in young children by destruction of the vasomotor and respiratory centres in the brainstem Ho et al, 1999;Huang et al, 1999;Komatsu et al, 1999;Lum et al, 1998;Wang et al, 1999).…”
