An Unusual Case of Spontaneous Bladder Perforation with Associated Autodialysis of the Ensuing Urinary Ascites

Abstract: Spontaneous rupture of the urinary bladder is a rare occurrence, and when encountered it is a diagnostic challenge. We present an unusual case of urinary bladder rupture in a patient with severe cerebral palsy who initially presented with localized abdominal pain and during admission developed generalized peritonitis caused by bladder rupture. In this case, the patient had none of risk factors associated with urinary bladder.

“…The most common cause of urinary bladder perforation is damage to the urinary bladder wall during massive abdominal trauma or iatrogenic, endoscopic excision of bladder tumours or bladder stones, during gynaecological and general surgical interventions. Other less common risk factors include bladder malignancy,2 diverticulum,4 cystitis,3 5 6 pelvic irradiation,7 diabetes mellitus,8 neuropathic bladder,9 alcohol excess,10 postpartum11 and chronic urinary outflow obstruction 12. Those cases that cannot be attributed to one of these are considered idiopathic, such as the patient reported here.…”

“…This phenomenon is called 'reverse autodialysis' and presents with 'pseudo renal failure'. 4 The diagnosis of bladder rupture is made with a cystography either conventional or CT assisted. Contrast of about 300 -400 ml is instilled into the bladder and radiographs done.…”

“…The new onset of renal failure in such clinical cases is based on the concept of "reverse auto-dialysis" of the peritoneal membrane, a reverse form of continuous ambulatory peritoneal dialysis that is most apparent when the presentation is delayed, thus leading to diffusion of metabolic waste products into the dialysate. The peritoneum subsequently reabsorbs urea and creatinine from the leaked urine, which increases the serum levels of these compounds (4,14).…”
