A 37-year-old woman at 17 weeks of gestation who was first noted to have proteinuria and microscopic hematuria at 13 weeks of gestation was admitted to our hospital with proteinuria that progressed to nephrotic syndrome (NS). Despite the treatment with prednisolone, including methylprednisolone pulse therapy, the NS worsened. The patient underwent an elective abortion at 21 weeks of gestation, and the NS then went into partial remission. A renal biopsy revealed membranous nephropathy (MN). There was no evidence of secondary MN. This is the first reported case of subclinical idiopathic MN that first developed in pregnancy.

Objetivo: reportar un caso de tumor del lecho placentario, cuya manifestación clínica fue síndrome nefrótico secundario a nefropatía IgA.Presentación del caso: se describe el caso de una paciente de 24 años, primigestante, quien fue remitida a una institución de cuarto nivel de complejidad localizada en Bogotá (Colombia), en donde se le hizo diagnóstico de nefropatía por IgA como manifestación paraneoplásica de un tumor trofoblástico del lecho placentario. Se realizó histerectomía total como tratamiento, con evolución satisfactoria, con resolución de los edemas y descenso de proteinuria; completó un año en seguimiento médico, sin evidencia de enfermedad.Discusión: el tumor trofoblástico del lecho placentario es infrecuente, hace parte del diagnóstico diferencial de la enfermedad trofoblástica gestacional (ETG), y puede manifestarse con fenómenos paraneoplásicos a nivel renal, que generalmente se resuelven con el tratamiento del tumor. La nefropatía por IgA puede ser secundaria a enfermedades inflamatorias crónicas y a patologías neoplásicas, como en este caso.

Invasive mole (IM), a malignant tumor, is a form of gestational trophoblastic neoplasia (GTN), which are characterized by invasive hydatid tissue into the myometrium or distant metastasis [1]. The most common transfer locations for IM are the vagina, lungs and brain. The most common symptom of IM is irregular vaginal bleeding, but further symptoms caused by bleeding in the metastases may also be detected, such as hemoptysis and neurological symptoms [2]. Myometrial invasion, swollen villi and hyperplastic trophoblast are often considered to be the pathological features of IM. IM′s clinical diagnosis mainly depends on medical history, clinical symptoms, laboratory tests and examination using imaging. Pathological results are the most essential basis for diagnosis. Good prognosis based on timely and comprehensive chemotherapy [3]. In this report, we describe a patient presenting with NS in which the underlying diagnosis of IM was made by a combination of chance and a high level of clinical suspicion, and to improve patient care amongst internists by heightening awareness of this uncommon condition.
