Analysis of antimicrobial susceptibility and virulence factors in Helicobacter pyloriclinical isolates

Abstract: Background: In this study, we evaluated the prevalence of primary resistance of Brazilian H. pylori isolates to metronidazole, clarithromycin, amoxicillin, tetracycline, and furazolidone. In addition, the vacA, iceA, cagA and cagE genotypes of strains isolated from Brazilian patients were determined and associated with clinical data in an effort to correlate these four virulence markers and antibiotic resistance.

“…Its effectiveness basically depends on rates of resistance to clarithromycin and is not recommended in areas where these rates among the population are higher than 20%. The few Brazilian studies on clarithromycin resistance show rates below this value (45,78,123,160) . While not outstanding, in the last 10 years this regimen has shown eradication rates close to 80% among us (29,54,134,166) .…”

“…Amoxicillin resistance is considered rare in most countries, but it is the most prescribed antibiotic for children in Brazil and up to 38% H. pylori strains have been reported to be resistant to this antibiotic in our country. 6 Metronidazole is a suitable alternative in countries with less than 40% of resistance to the drug, but in Brazil resistance to metronidazole is reported in 55% of the strains. 1,6 Tetracyclines and furazolidone are old antimicrobial drugs, and nowadays they are not frequently prescribed in children.…”

They are cheaper than traditional H. pylori treatment drugs, and the resistance rate of H. pylori to these drugs is lower than that reported to clarithromycin and metronidazole.…”
