Background and Aims: Several studies have attempted to identify patients at risk of developing severe pain after caesarean section (CS) by utilizing preoperative experimental pain application and clinical tests. The three-item questionnaire and reported pain intensity on infiltration of local anesthetic (LA) on the back of patient just before administration of spinal anesthesia, are two simple tests previously shown to be promising. We aimed to study utility of these two tools in Indian patients undergoing CS and find their correlation with postoperative pain and analgesic consumption. Material and Methods: A total of 150 parturients undergoing elective CS were enrolled. Preoperatively patients were asked to rate their level of anxiety, anticipated postoperative pain and analgesic need after surgery (three-item questionnaire). The pain intensity reported by patient upon LA injection for spinal anesthesia were recorded. In the postoperative period, pain intensity at rest, evoked pain and need for rescue analgesics were recorded. The correlation between three item questionnaire and pain on LA infiltration to postoperative pain were evaluated. To see relationship between the predictor variables to outcome, a multiple regression analysis was performed. Results: The predictors variables and postoperative pain were found to have mild correlation (r = 0.124 to 0.239). The predictor variables were significantly correlated with postoperative pain at rest but their association was not significant to evoked pain intensity. Multiple regression analysis showed that change in the predictors explains only 7-8% variance in postoperative pain outcomes. Conclusion: The three -item questionnaire and pain intensity reported upon LA infiltration for spinal anesthesia have mild correlation to postoperative pain in Indian parturients undergoing CS. As these variables predicts only 8% variance in pain experienced after CS, further studies are required for accurate prediction and targeted treatment of post CS pain.

Background: Cesarean section (CS) is the most common performed obstetric surgery world widely. Repeat CS is associated with a variety of complications, including intra-peritoneal adhesion, placenta previa, uterine rupture and cesarean hysterectomy. The present study aimed to determine the characteristics, maternal and fetal outcomes and post-operative complications of pregnant women with repeat CS using Clavien-Dindo classification in the Central Vietnam. Methods: We conducted a prospective study on 1342 women who underwent repeat CS between June 2020 and October 2021 at the Department of Obstetrics and Gynecology, Hue University Hospital, Hue, Vietnam. Numerous risk factors and adverse pregnancy outcomes of repeat CSs were identified and analyzed. Results: Intra-operative complications occurred at a rate of 18.2%, whereas post-operative complications rate was 2.8%. There were statistically significant differences in intra-operative complication rates between the pregnant women who had previously undergone only one CS and those who had previously two or more CSs (p = 0.011, 95% CI 1.9: 1.1–2.9). Among 1342 women who were indicated for repeat CS, thirty-seven women suffered from complications after surgery, accounting for 2.8%, as categorized by Clavien-Dindo. In which grade I accounted for 2.0%, grade II accounted for 0.4%, grade IIIa accounted for only 0.3%. No maternal mortality was reported in this study. Conclusions: Post-operative complications rate of cesarean section is extremely low. Application of the Clavien-Dindo classification in clinical scenario to classify those complications has shown to be highly practical.

Background: Over the past few decades, the indications for Caesarean delivery have broadened thus increasing the rate of caesarean section beyond the set level of WHO. The indications vary in different health care facilities. It is essential to determine the factors responsible for increasing caesarean section rates. To Objectives: nd out the cesarean section rate based on urgency as per RCOG and gestational age as per the categorization of pregnancy by ACOG in a tertiary health care facility catering to large South Indian population. It also aimed to determine the associated conditions contributing for caesarean section. Retrospective analysis of women who underwent caesar Methods: ean section from Jan-2017 to June 2017. Data was analyzed with respect to gestational age, Urgency of caesarean section, maternal fetal indications, clinical characteristics and associated conditions. Neonatal outcomes measured were gestational age, birth weight and NICU admissions. Data was expressed as proportions and percentages. The CS Results: rate was 24.5% among7, 346 deliveries and 81% were emergency CS. Preterm Caesarean sections were done in 18% and term 81.5%. Category II CS were the commonest (49%) followed by Category I (32%). Scarred uterus was the most common indication (85%) for elective caesarean section and fetal distress was most common indication (55.7%) for emergency caesarean sections. The associated factors were prior Caesarean section (32%), Medical disorders in 21% and 2.4% prolonged infertility and ART conceptions. Obstetric conditions including mal presentation were responsible in 19%. On the whole fetal indications accounted for 62%. Conclusion: Fetal indications, especially fetal distress is the most common reason for caesarean section which may have contributed for increased rate of caesarean section. Hence following strict criteria for diagnosis of fetal distress and adopting policies for intrauterine resuscitation prior to decision making for cesarean section would contribute to reduction of CS rate at tertiary health care centers.
