Analysis of Eyelid Fat Pad Changes With Aging

Abstract: With aging, there appears to be a pattern of differential fat pad alteration. This study shows that in the upper eyelids of patients >70 years of age, the medial fat pad becomes prominent whereas the central fat pad atrophies. Teleologically, the relative preservation of nasal fat may be due to its higher abundance of neural-crest progenitor cells. Clinically, this finding has implications in esthetic and functional upper eyelid blepharoplasty.

“…Lipoatrophy and volume loss are now recognized as significant components of upper lid and brow aging [1][2][3][4] Traditional upper lid blepharoplasty techniques that focus on soft tissue excision of skin and fat do not treat volume loss. 5 Therefore, recent technical advances aim to provide volume restoration for upper periorbital rejuvenation in select patients.…”

“…The central fat pad, namely the preaponeurotic fat pad, shifts posterosuperiorly, which may give an aged appearance in the central superior sulcus (Fig. 1) [2]. In the lower eyelid, protrusion of nasal, central, and temporal fat pads occur.…”
