Anatomic and Ultrasonographic Evaluation of the Knee Sensory Innervation

Abstract: We conclude that reproducible correlations showing the sensory innervations for the knee are linked to muscular structures. However, high variability among individuals makes it difficult to predict their paths. Our systematic approach, using direct visualization via ultrasound, allows a more accurate placement of the needle for therapeutic purposes.

“…Finally, the length, width and thickness of the semitendinosus, gracilis, sartorius tendons were measured, as well as the distance between the intersection and the tibial attachment of pes anserinus. We repeated examination procedures twice by the same technician to ensure consistency of the data [ 15 19 ] (as shown in Fig. 2 ).…”

“…[ 11 ] However, localising the genicular nerves can be difficult because of anatomical variations. [ 12 ],[ 13 ] Added to this, the procedure can be distressing when during localisation of genicular nerves the RF cannula comes in contact with pain-sensitive structures such as the periosteum and ligament insertion sites. [ 14 ] These limitations can be partly overcome using bipolar RF.…”

“…[ 18 ] Recently, ultrasound guidance has been used to locate the genicular nerves with improved success rates even in the situation of tremendous variabilities in the anatomy of the nerves. [ 12 ],[ 19 ] However, both techniques require sophisticated equipment and have a learning curve before one becomes equipped with the procedure.…”

“…[ 1 2 3 ] to regional blocks like the Femoral, sciatic, Adductor, combined femoral and sciatic[ 4 5 6 ] for targeting pain free TKA has paved way to more and more research into the subject. As the knee joint is supplied by various nerves (genicular nerves) from the femoral, obturator and sciatic nerves[ 7 ] hence needs a comprehensive yet simple technique for complete analgesia postoperatively. In this series, the authors have revisited and modified Roy et al .…”
