Ancillary factors in the treatment of orofacial pain: A topical narrative review

Abstract: Summary Ancillary factors, not directly related to treatment, often play a significant role by affecting therapeutic outcome. A search of the literature was conducted including words related to the placebo phenomenon and orofacial diseases. Therefore, critical factors have been grouped into three major categories: (a) the natural course of the diseases; (b) the regression of the symptoms to their mean intensity; and (c) placebo response. This topical narrative review describes the elements mentioned above, pro… Show more

“…TMDs are often self-limited over time, and significant fluctuation of the symptoms can be observed [44,45]; therefore, we cannot determine if the changes observed in both groups can be specifically attributed to interventions or simply the passage of time. Future clinical trials should include a real control group that does not receive any intervention to determine the real effects of the therapy.…”

“…Orofacial pain is the pain caused by damage to the branches of the trigeminal nerve or neurogenic inflammation that spreads to the orofacial area [1,2]. It accounts for approximately 10% to 26% of the adult population; in 7-11% of this population, it is classified as chronic and constitutes a serious medical and social problem [3,4]. Although the exact etiopathogenesis of most orofacial pain complaints has not been fully elucidated, recent findings suggest that the endoopioid (EOS) and endocannabinoid (ECS) systems have similar pharmacological characteristics, are frequently co-localized and exert a strong analgesic effect mainly through activation of the µ -opioid (MOR) and cannabinoid (CBR) receptors [5][6][7][8].…”
