“…Kempf et al identified the unique type E subtype as angioinvasive infiltrates of medium‐sized CD30+ lymphoid cells within walls of veins and small arterioles in the mid‐deep dermis, containing eosinophils arranged interstitially or clustering around blood vessels 4 . Although CD8+ predominance is seen, numerous cases have reported variable predominance of CD8 to CD4 4,13,14 . Our patient showed deep perivascular and interstitial infiltrates of variably sized lymphocytes associated with fibrinoid degeneration of blood vessel walls, endothelial swelling, extravasated erythrocytes, and numerous CD30+ cells both interstitially and around the affected blood vessels, consistent with Kempf et al's description.…”

“…A possible explanation for this could be CD4 predominance, as necrosis due to vessel destruction is prominent with expression of cytotoxic markers such as CD8 and TIA‐1 4,13 . This is similar to a reported case of a LyP type E in a 58‐year‐old male with a 45‐year history of a recurrent papular eruption without eschar formation, with histopathological findings of 10:1 CD4:CD8 predominance 14 . While the details of LyP associated malignancies is beyond the scope of this discussion, it is notable that compared to the development of malignancy in 20% to 40% of patients with LyP type A through D, in LyP type E they are reported to occur in only 5% of patients 8,10,12,13,17,28 .…”

“…The vast majority of cells found during immunohistochemical staining in material collected from persons suspected of type E LyP were T cells with expression of the CD30 receptor and a cytotoxic phenotype (CD8+/TIA-1) [ 64 ]. These cells dominated the vascular infiltrates, although CD4+ expressing cells were also sometimes involved [ 80 , 81 ]. Typically, these CD30+/CD8+ lymphocytes also express CD2 and CD5.…”
