Antegrade anterolateral thigh perforator flap advancement for soft‐tissue reconstruction of the knee: Case report

Abstract: Reconstruction of soft-tissue defects of the knee has always been a challenging task to the plastic surgeon. In some cases, local or regional flaps are too small or have limited arc of rotation for adequate coverage. Free flaps can be technically demanding and time consuming. We report for the first time an antegrade anterolateral thigh perforator flap advancement, used to reconstruct the knee soft tissue defect in a 54-year-old man. The operative procedure required skeletonizing the perforators of anterolater… Show more

“…In 2010, Fujiwara et al first described the use of ALT flap, as an advancement local unit, for the coverage of the knee region [15]. Even if the operative procedure required skeletonizing the perforators, the shorter operating time with decreased donor-site morbidity made this flap a valuable alternative for soft-tissue reconstruction of the knee.…”

“…Since his original report, ALT has become the workhorse perforator flap in pedicled soft tissue around the knee reconstruction. After, Fujiwara reported also a case of reconstruction performed by an ALT advancement flap based on the anterograde flow (Fujiwara, Chen, Ghetu, Jeng, & Kuo, ). However, reverse‐flow ALT flap in upper third of the leg reconstruction may often be complicated by vascular insufficiency because of the kinking of the pedicle during the insetting and in those cases a supercharged flap should be performed (Demirseren et al, ; Fujiwara et al, ; Rao et al, ).…”

“…Numerous surgical trials have been performed using musculocutaneous flaps, fasciocutaneous flaps, perforator flaps, and free flaps to repair these defects with varying degrees of success. [1][2][3] Local flaps created from the calf muscle are preferred primary surgical option. However, local flap may not be an option when there is injury to the donor site endangering the vascularity.…”
