A prospective observational study was conducted in a zonal care hospital probing the role of FNAC in palpable head and neck lesions & its diagnostic utility which can differentiate the various neoplastic and non neoplastic lesions of head and neck very cost effectively. To study diagnostic utility of fine needle aspiration cytology(FNAC) in head and neck lesions. A one year prospective observational study carried out at a zonal care hospital, which included a total of 120 patients reported with head and neck masses in the Otorinolaryngology OPD. Out of 120 FNACs, 38 (31.66%) cases were of thyroid origin, 35 (29.16%) cases included cervical lymph nodes, 34 cases (28.33%) arised from soft tissue and 04 (3.33%) from salivary glands. Thyroid lesion involvement (31.66%) was more common than any other lesion, out of which 84.21% were colloid goitre, 7.89% were Hashimoto’s thyroditis, 5.26% follicular neoplasm and 2.56% papillary carcinoma of thyroid. Amongst the lymph node swellings, 13 cases (36.11%) were reactive/ nonspecific lesion, 12 cases (33.33%) were tuberculous granulomatous lymphadenitis, 16.66% were cases of suppurative lymphadenitis. In malignant lesions of lymph nodes, metastatic deposit of carcinoma and poorly differentiated carcinoma were the predominant pathology and lymphoma case was nil. FNAC of head and neck swelling is a worldwide accepted diagnostic modality. It is simple, inexpensive and rapid method for the diagnosis of head and neck lesion and avoids unnecessary surgery in the form of excision biopsy for diagnosis.

Lipoma é um tumor benigno de origem mesenquimal com etiologia incerta, possui predileção por homens, entre os 50 e 70 anos de idade, sendo a região de tronco e extremidades os sítios mais comuns. Clinicamente, apresenta-se como lesão bem circunscrita, móvel, macia à palpação, indolor e de base séssil ou pedunculada. O diagnóstico é majoritariamente clínico com a confirmação por meio de análise histopatológica e por exames de imagens, ao qual o tratamento de escolha é a ressecção cirúrgica conservadora. Este trabalho tem por objetivo relatar o caso de tratamento cirúrgico de lipoma extenso na região submandibular em uma paciente do sexo feminino, 67 anos de idade, que apresentava aumento de volume na região submandibular direita. Ao exame clínico, apresentou lesão bem delimitada, móvel, macia à palpação e indolor. Foi preconizada a realização de uma biopsia excisional. Sob anestesia geral, realizou-se a incisão submandibular de Risdon, divulsão dos tecidos subcutâneos e hemostasia dos vasos sangrantes. A cirurgia seguiu com a ressecção da lesão, toalete da cavidade e sutura por planos. A peça patológica foi encaminhada para o Serviço de Anatomopatologia do Hospital das Clinicas da Universidade Federal de Pernambuco, medindo 14cm x 6,5cm, com confirmação da hipótese de Lipoma. O caso foi acompanhado por seis anos apresentando bom resultado estético e funcional sem recidiva. Portanto, mesmo que não muito frequentes na região de cabeça e pescoço, cabe ao Cirurgião Buco Maxilo Facial conhecer as características desse tipo de patologia, bem como a forma adequada de tratamento para prevenir recidiva.

Head and neck masses are classified as sebaceous cysts (epidermoid cysts), cervical lymphadenopathy, benign lipomas, lymph nodes, thyroid swellings, or tuberculosis lymphadenitis that may be painful or painless, adherent or fluctuant. In spite of this, they have distinct prognoses and pathological features. The anatomical location of the swelling and other demographic manifestations of the patient provide valuable information about the cause and type of swelling. Computed tomography (CT), magnetic resonance imaging, positron emission tomography-CT, and ultrasonography are the gold standard imaging methods for the head and neck examination. These methods are used according to the region considered for the study. Fine-needle aspiration cytology of lymph nodes is known to be effective, simple, and sometimes the only tool for the diagnosis of lymph node malignancies. This review highlights the epidemiological aspect of head and neck masses in the Indian population.
