Anterior Operation for Unstable Thoracolumbar and Lumbar Burst Fractures

Abstract: Titanium mesh cage filled with the autogenous cancellous bone shortened operation time and reduced the risk of complications in the donor site compared with the group that used the tricortical iliac bone.

“…Compared with traditional anterior surgery, only one rather than three levels of segmental vessels are severed in this improved procedure. Two Cobb elevators are used to press the segmental artery before cutting it, after which the two vessel ends are sufficiently dissociated before ligaturing (proximal part) or electrocoagulating (distal part) them.…”

“…Problems with allograft bone include graft collapse, graft fracture, nonunion, and donor-site complications [7][8][9][10]. Anterior corpectomy with a titanium mesh cage (TMC) is a safe and effective surgical treatment for thoracolumbar fracture, offering better and more predictable results with fewer donor-site complications and earlier biomechanical stabilization than iliac bone grafting [10][11][12][13]. However, TMCs have a fairly large amount of subsidence with a large Young's modulus, which may affect the long-term maintenance of kyphosis [12].…”

“…Анализ многолетнего опыта использования переднего спондилодеза аутотрансплантатом и отдаленные результаты операций свидетельствуют о том, что межтеловой спондилодез только аутокостью является недостаточно эффективным [7,8]. Это связано с ограниченными стабилизирующими возможностями способа.…”
