Antibiotic Resistance of Helicobacter pylori Among Male United States Veterans

Abstract: H pylori resistance to clarithromycin increased between 2009 and 2013; resistance to metronidazole remains high in infected men in the United States. The high frequency of resistance to levofloxacin is a new and concerning finding.

“…Th ere is little data to guide clinicians on either fl uoroquinolone resistance rates in North America or the impact of resistance on the effi cacy of fl uoroquinolone-containing treatment regimens. Th e little data that exist, suggest that fl uoroquinolone resistance may be as high-or perhaps even higher-than clarithromycin resistance in North America ( 122,141,142 ). Levofl oxacin triple therapy for 10-14 days appears to provide a comparable alternative to clarithromycin triple therapy.…”

“…Although current large scale data on H. pylori antibiotic resistance in North America are unavailable, recent data from Houston suggest that clarithromycin resistance rates may now fall within that range ( 122 ). In the absence of local or even regional H. pylori antibiotic resistance data, it is very important to ask patients about previous exposure to antibiotics for any reason, particularly macrolides and fl uoroquinolones, as this provides a proxy for underlying H. pylori antibiotic resistance ( 123,124 ).…”

“…According to Graham et al [119] , in the presence of fluoroquinolone resistance treatment success with quinolone-containing therapy decreases and these results can be predicted using a formula. A report published in United States in 2015 determined that the prevalence of levofloxacin resistance was 31.3% (95%CI: 23.1%-39.4%) [120] and another study showed a high rate of quinolone resistance (50%) in Congo [121] . We calculated the effect of quinolone resistance on treatment success using the proposed formula, which indicated success rates of 87.6% and 73.5%, respectively [119,122] .…”

“…In Brazil, resistance to amoxicillin has been detected in 29% of strains recovered from post-treatment [39]. However, many studies from other countries have shown that all strains examined are susceptible to amoxicillin [42,43]. The clinical significance of amoxicillin susceptibility testing has not been yet documented.…”
