Antibiotic use in 530 French hospitals: results from a surveillance network at hospital and ward levels in 2007

Abstract: This multicentre survey provided detailed information on antibiotic use in a large sample of hospitals and wards, allowing relevant comparisons and benchmarking. Analysis of consumption at the ward level should help hospitals to target practice audits to improve antibiotic use.

“…The pleiotropic hypothesis of niche evolution indicates that evolution toward CM-CTX-M is unlikely, as alleles increasing fitness during exposure to extendedspectrum cephalosporins will inevitably cause a regression of fitness when exposed to clavulanate and vice versa. However, double resistances might emerge as a consequence of the selection of compensatory mutations in the bla CTX-M gene (26) or by coincidental acquisition by horizontal gene transfer of CTX-M enzymes and other ␤-lactamases providing inhibitor resistance, such as bla oxa-1 (14).
