Drugs with antihistamine action are the most commonly prescribed medication in daily dermatologic practice, both to adults and children. This article addresses new concepts of the role of histamine receptors (H1 receptors) and discusses the anti-inflammatory effects of these drugs. Second generation antihistamines differs from first generation because of their high specificity and affinity for peripheral H1-receptors. Second generation antihistamines are also less likely to produce sedation because they have less effect on the central nervous system. Although the efficacy of the various H1-antihistamines in the treatment of allergic patients is similar, even when comparing first-and second-generation drugs, these drugs are still very different in terms of their chemical structure, pharmacology and toxic properties. Consequently, knowledge of their pharmacokinetic and pharmacodynamic characteristics is essential for a better medical care, especially that offered to pregnant women, children, the elderly, and patients with comorbidities. Este artigo aborda os novos conceitos da função dos receptores de histamina (receptores H1) e discute os efeitos anti-inflamatórios dessas drogas. A segunda geração de anti-histamínicos difere da primeira geração devido a sua elevada especificidade e afinidade pelos receptores H1 periféricos e devido a seu menor efeito no sistema nervoso central, tendo como resultado menores efeitos sedativos. Embora a eficácia dos diferentes anti-histamínicos H1 (anti-H1) no tratamento de doentes alérgicos seja similar, mesmo quando se comparam anti-H1 de primeira e de segunda geração, eles são muito diferentes em termos de estrutura química, farmacologia e propriedades tóxicas. Consequentemente o conhecimento de suas características farmacocinéticas e farmacodinâmicas é importante para a melhor prática médica, especialmente em gestantes, crianças, idosos e doentes com comorbidades. ÖZOrtalama 40 haftalık gebelik süresi, beraberinde fizyolojik ve hormonal değişiklikleri barındırır. Burun tıkanıklığı ve rinit semptomları gebenin yaşam kalitesini ve uyku düzenini olumsuz etkileyen başlıca sorunlardır. Rinolojik sorunlar, gebelik öncesinde tanı almış (septum deviasyonu, alerjik rinit vb.) ya da gebelik süresince belirginleşen (gebelik riniti, rinosinüzit, vb.) nazal tıkanıklık ve burun akıntısı semptomlarıyla karakterizedir. Aslında dikkatli bir öykü, bize sorunun süresi ve şiddeti hakkında ipucu verebilir. Alerjik rinit tanısı almış bir hastada nazal steroid kullanım öyküsü ile septum deviasyonu tanısı sonrasında önerilmiş operasyon varlığıyla sorunun kaynağı belirlenebilir. Öyküde, burun tıkanıklığı ve horlama sorunu, süresi, gebelikle ilişkisi, gebelik öncesinde burun akıntısı, hapşırık, koku bozukluğu, operasyon öyküsü varlığı, kullanılan ilaçlar bize rinolojik semptomların etiyolojisinde anlamlı ipuçları verebilir. Sonuç olarak, gebelik öncesinde rinolojik sorunlar için tedavi planlanması, hastanın eğitilmesi ve bilgilendirilmesi rahat bir gebelik sürecinde olası rinolojik sorunların çözümü için anlamlı katkı sağlamaktadır.Anahtar kelimeler: Gebelik, rinolojik sorunlar, tanı ve tedavi ABSTRACT Gestational period which lasts approximately 40 weeks harbours physiological and hormonal alterations. Nasal congestion and symptoms of rhinitis are the primary problems that affect pregnant woman's quality of life and sleep pattern adversely. Rhinologic problems are characterized by nasal congestion and nasal drip that are diagnosed preconception (septum deviation, allergic rhinitis etc.) or become evident during pregnancy (pregnancy rhinitis, rhinosinusitis etc). In fact, a carefully taken history may give us some clues about the duration and severity of the problem. In a patient who was diagnosed with allergic rhinitis the source of the problem can be identified through a history of nasal steroid usage and the operation recommended after septum deviation was diagnosed.Consequently, planning therapy for the preconception rhinologic problems, training and informing the patient will make important contributions to the resolution of rhinologic problems in a comfortable gestation process. Women with severe or uncontrolled asthma are at higher risk for pregnancy complications and adverse fetal outcomes than women with well-controlled asthma. Recent evidence-based guidelines have concluded that it is safer for pregnant women with asthma to be treated pharmacologically than to continue to have asthma symptoms and exacerbations. According to the Asthma and Pregnancy Working Group (APWG) of the National Asthma Education and Prevention Program, optimal treatment of asthma during pregnancy includes treatment of comorbid allergic rhinitis (AR), which can trigger or aggravate asthma symptoms. In general, treatment of both asthma and AR during pregnancy should follow the same stepwise approach that is used in the general population. This article presents the specific recommendations from the most recent APWG report and from other systematic reviews about which asthma and allergic rhinitis drugs should be preferred during pregnancy. Of the corticosteroids, budesonide has the most data and is listed as Pregnancy Category B (no evidence of risk in humans). Other inhaled and intranasal corticosteroids have less data and are listed as Pregnancy Category C but may be continued during pregnancy if the patient's asthma was well controlled with the medication before pregnancy.
