Antimicrobial resistance was evaluated in Campylobacter jejuni isolated from 1,291 diarrheic people over a 15 year period (2004-2018) in Southwestern Alberta, a model location in Canada with a high rate of campylobacteriosis. The prevalence of resistance to chloramphenicol, clindamycin, erythromycin, and gentamicin was low during the examination period (≤4.8%). Resistance to tetracycline remained consistently high (41.6-65.1%), and resistance was primarily conferred by plasmid-borne tetO (96.2%). Resistance rates to ciprofloxacin and nalidixic acid increased substantially over the examination period, with a maximal fluoroquinolone resistance (FQR) prevalence of 28.9% in 2016. The majority of C. jejuni isolates resistant to ciprofloxacin (93.9%) contained a C257T mutation within the gyrA chromosomal gene. Follow up with infected people indicated that the observed increase in FQR was primarily due to domestically acquired infections. Moreover, the majority of FQR C. jejuni subtypes (82.6%) were endemic in Canada, primarily linked to cattle and chicken reservoirs; 18.4% of FQR isolates were assigned to three subtypes, predominantly associated with cattle. Study findings indicate the need to prioritize FQR monitoring in C. jejuni infections in Canada, and to elucidate the dynamics of the emergence and transmission of resistant C. jejuni strains within and from cattle and chicken reservoirs.

Proposals to update the harmonised monitoring and reporting of antimicrobial resistance (AMR) from a public health perspective in Salmonella, Campylobacter coli, Campylobacter jejuni, Escherichia coli, Enterococcus faecalis, Enterococcus faecium and methicillin-resistant Staphylococcus aureus (MRSA) from food-producing animals and derived meat in the EU are presented in this report, accounting for recent trends in AMR, data collection needs and new scientific developments. Phenotypic monitoring of AMR in bacterial isolates, using microdilution methods for testing susceptibility and interpreting resistance using epidemiological cut-off values is reinforced, including further characterisation of those isolates of E. coli and Salmonella showing resistance to extended-spectrum cephalosporins and carbapenems, as well as the specific monitoring of ESBL/AmpC/carbapenemase-producing E. coli. Combinations of bacterial species, food-producing animals and meat, as well as antimicrobial panels have been reviewed and adapted, where deemed necessary. Considering differing sample sizes, numerical simulations have been performed to evaluate the related statistical power available for assessing occurrence and temporal trends in resistance, with a predetermined accuracy, to support the choice of harmonised sample size. Randomised sampling procedures, based on a generic proportionate stratified sampling process, have been reviewed and reinforced. Proposals to improve the harmonisation of monitoring of prevalence, genetic diversity and AMR in MRSA are presented. It is suggested to complement routine monitoring with specific cross-sectional surveys on MRSA in pigs and on AMR in bacteria from seafood and the environment. Whole genome sequencing (WGS) of isolates obtained from the specific monitoring of ESBL/AmpC/carbapenemase-producing E. coli is strongly advocated to be implemented, on a voluntary basis, over the validity period of the next legislation, with possible mandatory implementation by the end of the period; the gene sequences encoding for ESBL/AmpC/carbapenemases being reported to EFSA. Harmonised protocols for WGS analysis/ interpretation and external quality assurance programmes are planned to be provided by the EU-Reference Laboratory on AMR.

Objetivo. Detectar la presencia del gen ermB asociado a la resistencia a macrólidos en cepas de Campylobacter spp. aisladas de pollos comercializados en Lima, Perú. Métodos. Se analizaron 120 muestras de piel de pollo provenientes de tres mercados de los distritos de San Martín de Porres (n = 30), Santa Anita (n = 20) e Independencia (n = 70), ubicados en la Provincia de Lima, Perú. Se realizó el análisis microbiológico de las muestras según las recomendaciones de la norma ISO 10272-1:2017. Para la confirmación por reacción en cadena de la polimerasa (PCR, por sus siglas en inglés) de género y especie, se utilizaron los cebadores (primers) 16-rARN y GlyA e hipO, respectivamente. Para evaluar la sensibilidad antibiótica, se utilizó el agar Müller-Hinton sangre al 5% con sensidiscos de azitromicina (15 μg) y eritromicina (15 μg). La detección del gen ermB en cepas con fenotipos resistentes se realizó mediante PCR convencional. Resultados. Se obtuvo un total de 117 muestras positivas (97,5%), de las cuales 100% fueron compatibles con Campylobacter coli (prueba de hipurato negativa) y confirmadas por PCR. La evaluación de resistencia antibiótica en placa para azitromicina y eritromicina dio como resultado 100% de cepas con fenotipo de resistencia a estos macrólidos, mientras que la PCR para la detección del gen ermB indicó un total de 62 positivas (53%), que fueron confirmadas por secuenciamiento. Conclusiones. Estos resultados demuestran que las carcasas de pollo comercializadas en mercados de Lima presentan contaminación por C. coli con una alta resistencia a macrólidos, lo cual puede ser atribuido a la presencia del gen ermB.
