Application of a novel liquid biopsy in patients with hepatocellular carcinoma undergoing liver transplantation

Abstract: Circulating tumor cells (CTCs) serves a primary function in metastasis and recurrence of hepatocellular carcinoma (HCC). In the present study, in order to evaluate the analytical performance and clinical value of the liquid biopsy-based platform, a novel integrated subtraction enrichment and immunostaining-fluorescence in situ hybridization (iFISH®) platform was applied to analyze CTCs in patients with HCC undergoing liver transplantation (LT). In total, 30 patients with HCC undergoing LT and 10 healthy volunt… Show more

“…Another study using this platform also failed to uncover significant associations between change in total CTCs or CTC subtypes and HCC recurrence in a cohort consisting of 47 patients who underwent liver transplantation [96] . Nevertheless, Xue et al [97] utilized an iFISH® platform to detect CTCs in patients undergoing liver transplantation and found that patients with preoperative iFISH-CTCs ≥ 5 in 7.5 mL of blood had significantly shorter recurrence-free survival than those with lower CTCs. Further large, multicenter studies are still needed to confirm the association between different molecular phenotypes of CTCs and HCC prognosis.…”

“…In patients undergoing LT, an independent association between recurrence-free survival after LT and preoperative CTCs count was demonstrated 136. Even in patients with low AFP levels, CTCs count still predicted outstanding recurrence rates 136. Nevertheless, no significant association was observed between HCC recurrence after LT and total CTCs change or CTCs subtype in another study,137 probably related to its insufficient statistical power due to the small sample size.…”

“…A CTCs count slope is also associated with disease progression,133 and the persistence of CTCs after a potentially curative therapy may be an independent prognostic factor 134,135. In patients undergoing LT, an independent association between recurrence-free survival after LT and preoperative CTCs count was demonstrated 136. Even in patients with low AFP levels, CTCs count still predicted outstanding recurrence rates 136.…”

“…( 58,59 ) However, data in the transplant setting remain controversial with 1 study showing shorter recurrence‐free survival after LT in patients with detectable CTCs compared with those without CTCs (6 versus 15 months). ( 60 ) Another study showed no definitive predictive capacity from CTC enumeration. ( 61 ) The main limitation of these technologies is the relatively low sensitivity and lack of reproducibility across different technologies.…”
