Application of International Consensus Meeting Criteria for Classifying Drug-Induced Liver Disorders

Abstract: Screening and diagnosis of drug-induced liver disorders depend on careful history taking and 5 specific biochemical liver tests. The evolution of the liver disorder induced by cefadroxil therapy probably was interrupted because of its early detection. Appropriate screening was done with the subsequent administration of new potentially hepatotoxic drugs.

“…28 Because the nine patients with liver damage in our study had undergone chemotherapy using other anticancer drugs (eg, CDDP) without any adverse events beyond grade 3, it is unclear whether these anticancer drugs affected the liver enzyme levels. Gefitinib is metabolized by CYP3A4 and is O-demethylated by CYP2D6 in the liver.…”

“…It is less suggestive if this decrease occurs during a period of 30 days from withdrawing the offending drug. In the case of elevated alkaline phosphatase (ALP) or total bilirubin, a decrease of 50% within 6 months is suggestive of DILI of cholestatic type [8,10,11,15]. The diagnosis of DILI is frequently overlooked for several reasons.…”

“…Predominant elevation of transaminases favors hepatocellular death, whereas bile duct damage is evidenced by an increase in alkaline phosphatase levels. When neither laboratory values predominate, the type of hepatic injury is ambiguous and is classified as a mixed pattern [8,16].…”

“…These are then commonly used to assess the possible health risk in humans [22]. The liver is a metabolically active organ responsible for many vital life functions, some of such primary functions include; bile production and excretion; excretion of bilirubin, cholesterol, hormones, and drugs; metabolism of fats, proteins, and carbohydrates; storage of glycogen, vitamins, and minerals; synthesis of plasma proteins, such as albumin, and clotting factors, as well as blood detoxification and purification [23]. Due to these important activities, the liver is one of the body's organs most subject to injury.…”
