Appropriate Result from an Inappropriate ICD Shock

Abstract: A case of inappropriate detection of an ungrounded external AC current by an implantable cardioverter defibrillator (ICD) is reported. The resultant ICD shock dislodged the patient from the electrical source, thus preventing injury or electrocution.

“…In fact, if noise reversion had not been programmed on, the patient may have had an inappropriate ICD shock without induction of ventricular fibrillation, terminating the electrocution. This has been reported in the past, where inappropriate shocks were induced by EMI without ventricular fibrillation, which aborted fatal electrocution by knocking the subjects away from the power source 10,11 …”

“…The 60-Hz interference was detected as VF causing him to receive a shock from his ICD, throwing him back and releasing the power line from his hand. Davis et al 29 later described a similar ICD–patient incident.…”

“…Detection of electromagnetic interference followed by inappropriate therapy by implantable cardioverter‐defibrillators (ICDs) has been well documented in several case reports 1,2 as well as in follow‐up studies 3 . This is a report of a case where electromagnetic interference induced ventricular tachycardia (VT), with appropriate detection and treatment by the patient's ICD.…”
