Background Osteoid osteomas consist of a nidus surrounded by reactive sclerotic bone. The diagnosis typically is based on imaging and clinical presentation involving nocturnal pain. Removal of the lesion is essential and currently is performed mainly with image-guided, minimally invasive techniques. We describe a case involving an osteoid osteoma of the acetabular fossa, treated with arthroscopy-assisted radiofrequency ablation. Case Description A 47-year-old woman presented with a 9-month history of right groin pain and limited motion. The CT and MR images showed synovitis around the ligamentum teres and a nidus of the acetabular fossa, surrounded by sclerotic bone and protruding from the inner part of the lamina quadrilateral. Synovectomy and debridement of the ligamentum teres were performed, followed by radiofrequency ablation of the osteoid osteoma under direct arthroscopic observation of the hip, avoiding resection of the normal bone around the nidus and preserving the integrity of the quadrilateral lamina and cartilage. The patient had complete pain relief the next day with minimal morbidity and rapid functional restoration. At the 22-month clinical followup, the patient was asymptomatic, and the CT and MR images obtained 1 year after surgery showed no pathologic signs or synovitis. Literature Review Our case was the fifth such case to be treated with hip arthroscopy and the first of these to our knowledge to be treated with the arthroscopy-assisted radiofrequency ablation technique. Purposes and Clinical Relevance Arthroscopy-assisted radiofrequency ablation is a combined treatment technique that may be used for intraarticular lesions of the hip that otherwise would require a difficult approach and jeopardize damage to cartilage and bone and also treat concomitant synovitis.

A novel technique of using both a navigation system and an endoscope in intra-lesional curettage of benign bone tumors enables safe and adequate tumor removal via a minimal access approach. We performed curettage of benign bone tumors in five consecutive patients (4 female, 1 male, mean age 31.4 years) using a commercial CT-based navigation system supplemented by visual guidance through a shoulder arthroscope. The bone defect was filled with bone cement in four patients and with artificial bone substitute in one patient. Mean follow-up time was 8.8 months (range: 7-12 months). Mean duration of surgery was 144 min (range: 120-165 min). Mean wound length of each portal site was 19.5 mm (range: 15-25 mm). All patients could achieve a full range of joint movement and walk unaided at 4 weeks post-surgery. No local recurrence was noted.

rEsUMOO tratamento dos tumores ósseos benignos (TOB) e lesões ósseas pseudotumorais (LOP) tem visto surgir novos medicamentos, como os bisfosfonatos de uso intravenoso, que têm mostrado bons resultados no controle das lesões da displasia fibrosa. O cisto ósseo aneurismático tem sido tratado com agentes esclerosantes com sucesso. Tratamentos adjuvantes permitem a realização de cirurgias que preservam a articulação e a função, com baixas taxas de recidiva. Têm sido mais utilizados o cimento ósseo (PMMA), o fenol, a crioterapia com nitrogênio líquido, a água oxigenada, o álcool etílico e a radioterapia. Entre os novos métodos de tratamento surgidos destaca-se a ablação tér-mica por radiofrequência e por laser, utilizada principalmente para tratamento do osteoma osteoide. A artroscopia permite a ressecção de lesões benignas intra-articulares e assiste na ressecção de tumores subcondrais. Um grande avanço foi a utilização de substitutos sintéticos do osso, que associam substâncias osteoindutivas com material osteocondutivo e têm apresentado resultados comparáveis aos do enxerto ósseo autógeno. Há uma tendência atual para tratamentos fechados, fazendo-se a injeção percutânea de matriz óssea desmineralizada (DBM) associada com sulfato de cálcio. O enxerto ósseo esponjoso autógeno permanece como o padrão ouro. O enxerto de fíbula vascularizado apresenta os melhores resultados para incorporação em lesões maiores e agressivas. Também o suporte cortical alogênico provê resistência estrutural aumentada nessas lesões mais agressivas. O aloenxerto liofilizado tem indicação para preencher defeitos contidos e para reforço do enxerto autógeno.
