Objective: Previous studies have shown tacrolimus monotherapy and dual therapy with tacrolimus and low-dose prednisone as effective treatment modalities in managing membranous nephropathy. However, few studies have compared these therapeutic regimens. We compared the safety and efficacy and relapse rates of following these treatment regimens in primary membranous nephropathy in this study. Methods: One hundred and thirty-two patients(a total of 302 patients were excluded) diagnosed with MN viarenal needle biopsy at the First Affiliated Hospital of Zhengzhou University between January 2015 and June 2019 were retrospectively analyzed. The patients were divided into two groups based on the treatment regimen: (1) tacrolimus and prednisone dual therapy (T+P group, n = 67) treatment group; and (2) tacrolimus monotherapy (T group, n = 65) or the control group. High titer (PLA2Rab>150 RU / ml) may affect the outcome of PMN patients and therefore may be confounding factors, we performed a subgroup analysis to eliminate the bias in the relationship between the treatment regimen and the outcomes. Nineteen patients had a high titer in the T + P group, and 21 patients in the T group had a high titer. Patients were treated for approximately 48 weeks; observations were noted. Results: The T group had a remission rate of 64.62%, while the T + P group had a remission rate of 88.06%; the mean remission times were 21.82±2.66 weeks and 10.80±1.91 weeks, respectively. The relapse rate for the T group was 23.08%, and that for the T + P group was 4.48%. The PLA2Rab levels and 24-hour urine proteinuria levels were significantly different between the two groups.There were no significant differences in serum creatinine levels or glomerular filtration rates between the groups. In addition, the groups had no significant differences in complications, including infection, gastrointestinal syndrome, hepatic dysfunction, hyperglycemia, osteoporosis, and new-onset hypertension. Conclusions: Tacrolimus and low-dose prednisone dual therapy is efficacious in managing MN and lowers the recurrence rate in clinical practice.

Цель. В неинтервенционной наблюдательной открытой многоцентровой программе ХРОНОГРАФ (Получение дополнительныХ данных о РаспрОстраненности сНиженнОй скорости клубочковой фильтрации и альбуминурии у пациентов с артериальной ГипеРтонией с или без сАхарного диабета 2 типа в Российской Федерации) изучить распространенность маркеров хронической болезни почек (ХБП). Материал и методы. У 1363 пациентов с артериальной гипертонией (АГ) и/ или сахарным диабетом (СД) 2 типа рассчитана скорость клубочковой фильтрации (СКФ) по формуле CKD-EPI и определена альбуминурия (АУ) по отношению альбумин/креатинин (А/Кр) в утренней порции мочи. Результаты. У пациентов с СД (n=779, 62,6% женщин, 63,8 года) по сравнению с пациентами без СД (n=584, 50,7% женщин, 60,5 года) чаще выявлялось снижение СКФ <60 мл/мин/1,73 м 2 (38 и 31%), <45 мл/мин/1,73 м 2 (C3б-С5) (15,92 и 9,94%) и <30 мл/мин/1,73 м 2 (C4-С5) (4,24 и 2,06%), высокая и очень высокая АУ (36,6 и 28,1%), очень высокая АУ (А3) -(7,32 и 2,40%). У пациентов с СД чаще выявлялось наличие хотя бы одного маркера ХБП (снижение СКФ <60 мл/мин/1,73 м 2 и/или А/Кр >30 мг/г) (54,0 и 44,35%), изолированное снижение СКФ <60 мл/мин/1,73 м 2 (17,46 и 16,27%), изолированное повышение А/Кр >30 мг/г (15,66 и 13,35%) и сочетание снижения СКФ и высокой/очень высокой АУ (20,93 и 14,71%). Среди пациентов с СД ниже пропорция лиц с низким комбинированным риском прогрессирования ХБП и сердечно-сосудистых осложнений (45,2 и 53,6%), выше -с очень высоким риском (28,5 и 20%). Наиболее частыми являются сочетания, соответствующие начальным стадиям ХБП: С3аА1, С2А2 и С3аА2. Заключение. В Российской Федерации пациенты с АГ независимо от наличия СД характеризуются высокой частотой маркеров ХБП. У пациентов с СД по сравнению с пациентами без СД чаще выявляли снижение СКФ <60 мл/ мин/1,73 м 2 , повышение А/Кр >30 мг/г как изолированное, так и в сочетании. Chronic obstructive pulmonary disease (COPD) has many systemic effects, one of which is impaired renal function. Existing studies prove that patients with COPD often have risk factors for developing chronic kidney disease (CKD). Many risk factors for developing COPD are common for the onset of CKD. However, in routine clinical practice, there is an underestimation of the incidence of renal dysfunction in COPD patients, while in-depth and targeted studies reveal changes in renal function in patients with COPD. Topical methods for identifying initial changes in renal function in COPD patients are the study of the glomerular filtration rate, the evaluation of the level of microalbuminuria, the study of the index of resistance of renal vessels. The importance of identifying CKD in the early stages in COPD patients is determined by the increased risk of cardiovascular events and the onset of cardiorenal syndrome in patients.
