Assessment of anti-cyclic citrullinated peptide in psoriatic arthritis

Abstract: BackgroundAnti-cyclic citrullinated peptides (anti-CCP) are highly specific diagnostic and prognostic markers for rheumatoid arthritis (RA). They have been also found in psoriatic arthritis (PsA), with controversies as regards clinical and radiological associations. The current study assessed anti-CCP in PsA and determined its clinical and radiological associations.MethodsFour groups contributed to this study. 40 PsA, 40 psoriasis without arthritis, 40 RA and 40 healthy controls. They were tested for anti-CCP.… Show more

“…A total of 119 studies met the selection criteria and were included. Of these, 19 studied clinical markers [ 18 36 ], 69 studied laboratory markers [ 27 , 37 , 38 , 40 55 , 57 73 , 75 – 96 , 124 133 ], and 32 studied genetic markers [ 97 113 , 115 123 , 134 139 ]. One study described both clinical and laboratory markers [ 27 ].…”

“…Of those 14 markers, six showed a positive association with the presence of PsA in Pso: the presence of anti-citrullinated protein antibodies (ACPA) [ 37 40 ], a higher level of IL-34 [ 56 , 66 ], a higher level of tumor necrosis factor alpha (TNFα) [ 56 , 64 ], a higher mean platelet volume (MPV) [ 68 , 69 ], a higher LDL:HDL ratio [ 62 , 64 , 71 , 76 ], and the presence of microRNA miR-146a-50 [ 78 , 79 ].…”

“…This rate is in good agreement with smaller studies of PsA patients, which have reported ACPA-positive rates ranging between 3.1 and 17.5% [ 5–15 ]. Although some studies [ 8 , 12 ], including a recent study from the CORRONA database (n = 958) [ 14 ], did not identify significant differences in measures of disease activity between ACPA-positive and ACPA-negative PsA patients, others have reported higher rates of erosive disease [ 7 , 9 , 10 ] and more involved joints [ 10 , 11 , 13 , 15 ] in PsA patients who were positive for ACPA. However, the conclusions of these studies are somewhat limited by their small size; the largest study reporting an association between ACPA seropositivity and disease activity included 192 PsA patients, of which 15 were ACPA-positive [ 11 ].…”

“…It is noted in the literature that these antibodies have significance in PsA for the evaluation of osteoporosis, erosive changes and bone destructiones -fractures. Data obtained for ACPA autoantibodies are not higher than previously tested kits from other researches [14][15][16][17][18][19]. The ACPA autoantibody as an isolated laboratory variable does not dominate with its performances in diagnose of early, undiferentiated PsA.…”
