Assessment of Cardiac Sarcoidosis with Advanced Imaging Modalities

Abstract: Sarcoidosis is a chronic systemic disease of unknown etiology that is characterized by the presence of noncaseating epithelioid granulomas, usually in multiple organs. Several studies have shown that sarcoidosis might be the result of an exaggerated granulomatous reaction after exposure to unidentified antigens in genetically susceptible individuals. Cardiac involvement may occur and lead to an adverse outcome: the heart mechanics will be affected and that causes ventricular failure, and the cardiac electrical… Show more

“…Autopsy findings have shown myocardial involvement in at least 25% of patients, which could be underestimated due to the microscopic nature of the involvement . Conclusive findings on endomyocardial biopsy can be pathognomonic for CS; however, most often have low diagnostic yield, largely due to its patchy distribution . Therefore, noninvasive tools have been extensively studied to identify sensitive parameters reflecting cardiac infiltration.…”

“…1 Conclusive findings on endomyocardial biopsy can be pathognomonic for CS; however, most often have low diagnostic yield, largely due to its patchy distribution. 15 Therefore, noninvasive tools have been extensively studied to identify sensitive parameters reflecting cardiac infiltration. On echocardiography, classic features of CS include regional wall motion abnormalities and wall thinning, usually of the basal anterior septum.…”

“…Myocardial granulomas are detected post-mortem in 12 to 25% of patients with extracardiac sarcoidosis [1,2]. These granulomas lead to subclinical myocardial inflammation, edema, fibrosis, and scars, which in turn result in ventricular remodeling and systolic dysfunction [3]. The prognosis of cardiac sarcoidosis (CS) is poor; a 5-year mortality rate of 25 to 40% in the advanced stages of CS is attributable to arrhythmias, restrictive cardiomyopathy, and sudden cardiac death [4,5].…”

“…The diagnosis of CS lacks reliable and specific tools, particularly during the early stages of the disease [24]. Cardiac magnetic resonance can facilitate both the diagnosis and assessment of response to treatment for cardiac sarcoidosis.…”
