Assessment of Rotational Instability with Disruption of the Accessory Collateral Ligament of the Thumb MCP Joint: A Biomechanical Study

Abstract: Objectives The objective of this paper was to biomechanically investigate rotational stability of the thumb after ulnar collateral ligament (UCL) and accessory collateral ligament (ACL) disruption and repair at the metacarpal joint of the thumb. Methods Twelve fresh frozen adult cadaveric thumbs were used. The torsion test was performed under constant rotation of 1/s through 30 arc of metacarpal phalangeal (MCP) joint. The torsional resistance was determined for four categories: first no intervention of the UC… Show more

“…The order of sectioning the tissues, pUCL, aUCL, and then the aponeurosis, reproduced the sequence of injury in traumatic events. 10,16) Similar to the finding of Adler et al, we were unable to create a Stener lesion after complete sectioning of the UCL at 0°and 30° of flexion with no rotation. 16) We also did not create a Stener lesion with supination or pronation with intact overlying fascia.…”

“…8) Acute injuries are the result of a sudden, significant radial force causing abduction of the thumb. [9][10][11] The results of stress testing of the thumb MCP joint help to determine whether treatment will be non-operative with immobilization or with surgical reconstruction or repair. Fluoroscopy, MRI and ultrasound are increasingly being used and evaluated as diagnostic tools.…”

“…The accessory collateral ligament lies palmar to the proper ligament and is in continuity with it and the volar plate [16,26]. The role of the accessory collateral ligament in thumb stability has recently been proven [9].…”

“…The ACL lies palmar to the UCL and attaches at the palmar plate and the ulnar sesamoid (Stener,1962; Fraser et al,2008; Anderson,2010). The UCLC is covered by the aponeurosis of the adductor muscle (Stener,1962).…”
