Assessment of surgical stress during general anaesthesia

Abstract: SSI reacts to surgical nociceptive stimuli and analgesic drug concentration changes during propofol-remifentanil anaesthesia. Further validation studies of SSI are needed to elucidate its usefulness during other anaesthetic and surgical conditions.

“…The Paedfusor pharmacodynamic model was used to obtain C p (t) and the DOH monitor was modeled as described in Section III-C. Fig. 3 shows the wakefulness measurement, WF 7 , from one of the cases together with the simulated WF measurement provided by the corresponding identified patient model. The simulation of WF was obtained by driving the identified patient model with the recorded propofol infusion rate from the actual case and subsequently applying the monitor model disclosed in Section III-C to the resulting signal 8 .…”

“…Closed-loop control of analgesia is limited by the current absence of reliable nociception monitors [6], although several measurement indices are being developed [7], [8]. However, nociception is reflected in the DOH measurement, as presented in Section III-C.2.…”

“…It creates a linear scale score between 0-100, with high readings suggested to represent nociceptive stimulation [22]. Both of the contributing measurements are thought to reflect the balance between sympathetic and parasympathetic tone, which is influenced by nociceptive stimulation.…”
