Abstract:The aim of this study was to assess the efficacy of an oral supplement (CP) containing collagen peptide, soy peptide, and chrysanthemum extract in Chinese female adult volunteers with melasma. The approval of the Institutional Ethics Committee of the third affiliated hospital, Sun-Yat Sen University, was obtained before the study. A signed consent was obtained from each volunteer prior to study to enable the volunteer to appreciate the aim of the study and the consequences of her consent. Sixty-two female volunteers aged 30-60 years were included in the study, and were randomized into a treatment group or a placebo group. The skin tone of the pigmented spots was evaluated using Chromameter, and pigment density was evaluated using Mexameter before and after the treatment. Significant changes in skin tone parameters of L value and ITA • (individual typology angle) were detected in the lesion area after the treatment (P < 0.01). When compared with placebo group, the treatment group achieved significant improvement in the brightness of the pigmented spots at the 45 and 60-day time points. A significant decrease in the level of melanin was observed in the treatment group when compared with the placebo group (p < 0.01). All data demonstrated through non-invasive in vivo instrumental measurement that daily oral intake of CP had clinical efficacy of reducing melasma severity.

ÖZ GİRİŞDeriye rengini keratinositler ve melanositlerdeki melanin pigmenti verir. Bunun yanında, stratum korneumun kalınlığı, damar yapısı, karotenoid gibi eksojen pigmentlerin varlığı da etkilidir. Deri hiperpigmentasyonu, melanositlerin sayı ve fonksiyonlarındaki artış ya da deriye renk veren diğer maddelerin birikimi sonucu ortaya çıkar (1) .Hiperpigmentasyon epidermiste, dermiste ya da her iki alanda melanin miktarındaki artışı tariflemek için kullanılan bir terimdir. Birçok faktör fasiyal hiperpigmentasyon nedeni olabilmekte ve pigment artışı lokal bir durum olabileceği gibi jeneralize bir hastalığın komponenti olarak da karşımıza çıkabilmektedir (2) .Hiperpigmentasyon, hasta yaşam kalitesini önemli ölçüde etkileyen kozmetik bir sorundur. Çoğunlukla orta yaş kadın hastaları etkileyen bu patoloji dermatologlar için ciddi tedavi güçlüğü oluşturan bir antitedir (3) . Her ne kadar melazma fasiyal hiperpigmentasyon en sık nedeni olarak karşımıza çıksa da, ashy dermatozu, Riehl'in melanozisi, Civatte'nin poikiloderması, Brock'un peribukkal eritrozisi, ilaçla indüklenen ve postinflamatuar hiperpigmentasyon gibi birçok formu mevcuttur (4,5) .Bu makalede, kliniklerimizde en sık görülen kazanılmış hiperpigmentasyon nedenleri olan melazma, postinflamatuar hiperpigmentasyon (PİH), efelid ve lentigolar üzerinde durulacaktır. MELAZMADerinin en sık karşılaşılan hipermelanozisi olan melazma, güneş maruziyeti olan deride (özellikle yüzde) gelişen, rengi açık kahveden koyu kahve tonlarına değişebilen, düzensiz sınırlı yama ve maküller ile karakterizedir (6) . Epidemiyoloji ve EtiyolojiMelazmanın gerçek insidansı bilinmemektedir. Kadınlarda daha yaygındır. Bildirilen olguların yalnızca %10'unu erkekler oluşturmaktadır. Özellikle 3.-4. dekatta sıktır. SummaryBy midcentury, the U.S.A. will be more ethnically and racially diverse. Skin of colour will soon constitute nearly one-half of the U.S. population, and a full understanding of skin conditions that affect this group is of great importance. Structural and functional differences in the skin, as well as the influence of cultural practices, produce variances in skin disease and presentation based on skin type. In the skin of colour population, dyschromia is a growing concern, and a top chief complaint when patients present to the physician. A thorough understanding of the aetiology and management strategies of facial hyperpigmentation is of importance in caring for those afflicted and also in the development of new therapies.What's already known about this topic?• In the skin of colour population, facial hyperpigmentation is a common and growing concern when presenting to the physician.• Facial hyperpigmentation can cause significant cosmetic disfigurement with subsequent emotional impact. Therapy continues to be challenging as there is no universally effective treatment. Existing agents have varying degrees of efficacy and potential risk of postinflammatory hyperpigmentation with different treatment protocols.What does this study add?• Persons of colour will soon comprise a majority of the international and domestic populations.• A comprehensive knowledge and approach to assessment and treatment is necessary to care properly for skin of colour patients.• This review thoroughly discusses aetiologies of facial hyperpigmentation and categorizes appropriate treatment strategies.Skin of colour, also known as ethnic skin, constitutes a wide range of racial and ethnic groups traditionally referring to persons of African, Asian, Native American, Middle Eastern and Hispanic backgrounds. By 2050, nearly one-half of the U.S. population will be nonwhite.1 These skin types are generally categorized as Fitzpatrick types IV-VI, and are more richly pigmented. Structural and functional differences in the skin, as well as the influence of cultural practices, produce variances in skin disease, presentation and treatment based on skin type. Darker skin phenotypes are characterized by higher content of melanin, higher eumelanin to pheomelanin ratio, and more effective distribution of melanin for protection against ultraviolet (UV) radiation. 2In skin of colour, the amount and epidermal distribution of melanin is an important biological feature.
